And despite the wholesome-food movement’s frequent and inexcusable obliviousness to the obesogenicity of many of its own foods, it deserves credit for paying more attention to those healthier ingredients than does Big Food.

Hold on, you may be thinking. Leaving fat, sugar, and salt aside, what about all the nasty things that wholesome foods do not, by definition, contain and processed foods do? A central claim of the wholesome-food movement is that wholesome is healthier because it doesn’t have the artificial flavors, preservatives, other additives, or genetically modified ingredients found in industrialized food; because it isn’t subjected to the physical transformations that processed foods go through; and because it doesn’t sit around for days, weeks, or months, as industrialized food sometimes does. Several areas of research are exploring mechanisms that link obesity and cancer (29, 46). One research area involves understanding the role of the microbes that live in the human gastrointestinal tract (collectively called the gut microbiota, or microbiome) in both type 2 diabetes and obesity. A group in Amsterdam, meanwhile, is investigating whether transferring feces from lean to overweight people will lead to weight loss. U.S. researchers tend to view such “fecal transplants” as imprecise and risky. Even so, a number of scientists are actively developing potential treatments. Dominguez-Bello, for example, is conducting a clinical trial in Puerto Rico in which babies born by cesarean section are immediately swabbed with a gauze cloth laced with the mother’s vaginal fluids and resident microbes. Weight-loss medication choices are more limited in older adults. This shortens the list of available medications for weight-loss. Side effects, existing medical conditions and interactions with other medications are the major barriers in prescribing weight-loss medications in the elderly. Bariatric surgery is being increasingly considered in older adults as well. The existing medical problems, surgical risk and benefits from the surgery need to be closely analyzed by the medical team and discussed with the patient to ensure an optimal decision and a satisfactory outcome.
HCA reduces the syntheses of fat from carbohydrates by 40-70% and forces glucose to form additional glycogen. As glycogen levels increase, the body sends a satiety (“full”) signal to the brain, which decreases appetite and food intake. In animal studies, HCA reduced food intake by about 10%.
