At McDonald’s, “Chef Dan”—that is, Dan Coudreaut, the executive chef and director of culinary innovation—told me of the dilemma the movement has caused him as he has tried to make the menu healthier. “Some want us to have healthier food, but others want us to have minimally processed ingredients, which can mean more fat,” he explained. “It’s becoming a balancing act for us.” That the chef with arguably the most influence in the world over the diet of the obese would even consider adding fat to his menu to placate wholesome foodies is a pretty good sign that something has gone terribly wrong with our approach to the obesity crisis.
Identifying and avoiding food triggers. Distract yourself from your desire to eat with something positive, such as calling a friend. Practice saying no to unhealthy foods and big portions. Eat when you’re actually hungry — not simply when the clock says it’s time to eat.

And while cutting back on the calories can be an important part of the weight loss process, cutting back on the right calories may be even more important. As registered dietician and diabetes educator Amy Campbell told US News and World Report, “One concern for older people is getting enough protein. We need more as we age.” The takeaway? It’s not necessarily about following a specific diet, says Campbell, but instead about choosing a “well-rounded diet with extra protein.”
While losing weight is a simple concept in theory — expend more calories than you take in — it is a more complex process in reality. But for seniors, the “battle of the bulge” can be even harder to the natural slowing down of the metabolism. Several areas of research are exploring mechanisms that link obesity and cancer (29, 46). One research area involves understanding the role of the microbes that live in the human gastrointestinal tract (collectively called the gut microbiota, or microbiome) in both type 2 diabetes and obesity. Both conditions are associated with dysbiosis, an imbalance in the collection of these microbes. For example, the gut microbiomes of obese people are different from, and less diverse than, those of non-obese people. Imbalances in the gut microbiota are associated with inflammation, altered metabolism, and genotoxicity, which may in turn be related to cancer. Experiments in mice show that the microbiome may influence the efficacy of some types of cancer treatment, particular immunotherapy (47, 48). Researchers are beginning to think about ways to change the microbiota of cancer patients to improve their outcomes.
In addition to its health impacts, obesity leads to many problems including disadvantages in employment and increased business costs. These effects are felt by all levels of society from individuals, to corporations, to governments.
“For the baby boomers, nothing will magically happen to change that,” Hamburg said. “Societal change will take time. It’s a matter of education and realizing the danger of going into retirement age with a less healthy life and more chronic disease. For the time being, the boomers are aging into obesity-related illnesses, which will translate into a cost crisis for health care and Medicare.”

Back in February, when I was at Real Food Daily in Los Angeles, I ordered the “Sea Cake” along with my green-vegetable smoothie. It was intensely delicious in a way that set off alarm bells. RFD wouldn’t provide precise information about the ingredients, but I found a recipe online for “Tofu ‘Fish’ Cakes,” which seem very close to what I ate. Essentially, they consist of some tofu mixed with a lot of refined carbs (the RFD version contains at least some unrefined carbs) along with oil and soy milk, all fried in oil and served with a soy-and-oil-based tartar sauce. (Tofu and other forms of soy are high in protein, but per 100 calories, tofu is as fatty as many cuts of beef.) Genetics. A person is more likely to develop obesity if one or both parents are obese. Genetics also affect hormones in fat regulation. For example, one genetic cause of obesity is leptin deficiency. Leptin is a hormone produced in fat cells and also in the placenta. Leptin controls weight by signaling the brain to eat less when body fat stores are too high. If, for some reason, the body cannot produce enough leptin or leptin cannot signal the brain to eat less, this control is lost, and obesity occurs. The role of leptin replacement as a treatment for obesity is currently being explored.
A baseline evaluation for unexplained, unintentional weight loss in older adults includes history, physical examination, laboratory tests, chest radiography, fecal occult blood testing, and possibly abdominal ultrasonography.
Some states will be harder hit than others. Colorado, for example, can expect the numbers of older people with diabetes to increase by 138 percent by 2030, while Arizona will see its population of obese people over 65 grow by 90 percent.
