Sugar drinks are the largest source of added sugar in the diets of children and adolescents. Increasing consumption of these high caloric beverages that offer little or no nutrients is associated with the increasing rates of childhood obesity.

Weight loss occurs when more calories are expended than are consumed. Healthy animals can experience weight loss, but in a geriatric cat, a subtle decrease in weight can also be the first indication of illness. For example, cats with small intestinal disease may lose weight before exhibiting anorexia, vomiting, or diarrhea. Unfortunately, weight changes in older cats are often attributed merely to aging, so clients may not seek veterinary care or veterinarians may inadvertently delay a diagnostic workup until marked weight loss is evident or additional clinical signs arise. Bioelectric impedance analysis (BIA): There are two methods of the BIA. One involves standing on a special scale with footpads. A harmless amount of electrical current is sent through the body, and then percentage of body fat is calculated. The other type of BIA involves electrodes that are typically placed on a wrist and an ankle and on the back of the right hand and on the top of the foot. The change in voltage between the electrodes is measured. The person’s body fat percentage is then calculated from the results of the BIA. Early on, this method showed variable results. Newer equipment and methods of analysis seem to have improved this method.

For females, a waist circumference of 35 inches or greater is considered unhealthy. For men, a waist circumference of 40 inches or greater is considered unhealthy. They face potentially shortened life spans – and an overburdened health care system faces additional spikes in cost.

You probably will need to gradually increase the amount you exercise as your endurance and fitness improve.

Some people have tried combining more than one weight-loss drug or combining a weight-loss drug with other drugs for the purposes of losing weight. The safety and effectiveness of such drug “cocktails” is not known.

Three papers (Villareal 2011a; Armamento-Villareal 2012; Shah 2011) reported on a cohort of 107 frail obese adults using similar inclusion criteria and interventions to their earlier trials. In Villareal 2011a, weight loss plus exercise improved physical function and ameliorated frailty more than either weight loss or exercise alone, and sarcopenic-obesity was reduced in all intervention groups. However, there was a loss of both lean body mass and hip BMD. These loses were attenuated by the addition of exercise but not stopped. It is currently unknown what additional intervention(s) are needed to totally mitigate these loses. The investigators suggested higher doses of calcium and vitamin D, or performing only aerobic or resistance exercise individually, or including anti-resorptive therapy during active weight loss might be effective in mitigating the losses of BMD and LBM during active weight loss.

Gallbladder cancer: Compared with normal-weight people, people who are overweight have a slight (about 20%) increase in risk of gallbladder cancer, and people who are obese have a 60% increase in risk of gallbladder cancer (19, 20). The risk increase is greater in women than men.

First of all, hats off to your mom. She’s got her hands full. So when she gets upset, everyone should be understanding to her plite. And, your father should be appreciative of the sacrifices she’s making in time and energy to care for him. I think he should be made fully aware of the issues he’s creating in the rest of the family’s lives as well. Especially your mom’s as primary caregiver. Let him know that you love him and care about his well being, but that he is not alone in this equation. He has to consider others. Also let him know what alternatives are be explored, if no changes can be made. For health purposes, I suggest cleaning out the cabinets and refrigerator of unhealthy foods and replacing with healthy alternatives (lots of vegetables and fruits). Maybe explore different recipes to make eating healthy more appealing. If he’s in a wheelchair and not very mobile, he probably won’t be going shopping for food. Thus, his caregiver is in charge. He may grumble for a short while, but it will be well worth it in the long run. His attitude may even change for the better, because he will start to feel better about himself.

Finally, cumulative attrition of the most vulnerable fraction of the obese population brought about by premature mortality of those subjects who do not survive the late-midlife years leaves only the most biologically advantaged obese survivors for “nonbiased” epidemiological analysis of obesity in advanced years. If one accepts that obesity increases mortality in younger years, attempted comparison between age-matched obese and lean humans in the older age inevitably leads to the study of two highly unequal cohorts of which only one has been subjected to the Darwinian process of obesity-related attrition.

“The problem with using only primary care providers,” says Bonnie Modugno, a registered dietician in Santa Monica, Calif., “is that they completely ruled out direct reimbursement for the population of providers who are uniquely qualified and experienced working with weight management. I think that was a big mistake.”

The U.S. Food and Drug Administration approved orlistat capsules, branded as alli, as an over-the-counter (OTC) treatment for overweight adults in February 2007. The drug had previously been approved in 1999 as a prescription weight loss aid, whose brand name is Xenical. The OTC preparation has a lower dosage than prescription Xenical.

Another job vacancy associated with obesity might be one normally filled by a stomach bacterium called Helicobacter pylori. Research by Martin Blaser of New York University suggests that it helps to regulate appetite by modulating levels of ghrelin—a hunger-stimulating hormone. H. pylori was once abundant in the American digestive tract but is now rare, thanks to more hygienic living conditions and the use of antibiotics, says Blaser, author of a new book entitled Missing Microbes.

Ben Sheidler, a spokesman for Coca-Cola, said the company had created 32 new beverages in the last 18 months, and that 65 percent of its drinks portfolio in Chile could now be described as having low or reduced sugar.

“We’re all creatures of habit,” Campbell says. So, she says, imagine you’re 75 years old and have to change your habits and incorporate new foods like tofu. Although most diets offer plenty of online and printed resources, they can be overwhelming. “It’s hard sometimes to pick up a book and say, ‘what should I be eating,'” she says. For older adults, it can help to work with dietitians.

The interaction between diet and gut bacteria can predispose us to obesity from the day we are born, as can the mode by which we enter the world. Studies have shown that both formula-fed babies and infants delivered by cesarean section have a higher risk for obesity and diabetes than those who are breast-fed or delivered vaginally. Working together, Rob Knight of the University of Colorado Boulder and Maria Gloria Dominguez-Bello of N.Y.U. have found that as newborns traverse the birth canal, they swallow bacteria that will later help them digest milk. C-section babies skip this bacterial baptism. Babies raised on formula face a different disadvantage: they do not get substances in breast milk that nurture beneficial bacteria and limit colonization by harmful ones. According to a recent Canadian study, babies drinking formula have bacteria in their gut that are not seen in breast-fed babies until solid foods are introduced. Their presence before the gut and immune system are mature, says Dominguez-Bello, may be one reason these babies are more susceptible to allergies, asthma, eczema and celiac disease, as well as obesity.

Gallbladder disease. Obesity is a major risk factor for gallstones because obesity is believed to reduce the amount of bile salts in bile, resulting in more cholesterol. Additionally, gallbladder emptying is decreased as a result of obesity (National Digestive Diseases Clearinghouse, 2004). Again, management of obesity, as described below, is the primary approach for decreasing the incidence of this gallbladder disease.
Obesity is an epidemic in the United States and in other developed countries. More than two-thirds of Americans are overweight, including at least one in five children. Nearly one-third are obese. Obesity is on the rise in our society because food is abundant and most of us are employed in positions that require little to no physical activity. As many as 85% of dieters who do not exercise on a regular basis regain their lost weight within two years. In five years, the figure rises to 90%. The primary factor in achieving and maintaining weight loss is a life-long commitment to regular exercise and sensible eating habits.
What can you do right now to stop this dangerous trend? It’s simple. When I saw the post-menopausal pounds creeping up around the time I turned 50 a few years ago, I took action: I started to walk, then run. Push-ups and sit-ups became part of my daily life, and I began a healthy eating program which is comprised of eating small, healthy meals every two to three hours. The combination of moving my body every day and eating less was all it took. At the age of 54, I am fitter than I’ve ever been. There isn’t a single thing I do that any other average American couldn’t do, too.
The high rates of obesity and depression, and their individual links with cardiovascular disease, have prompted many investigators to explore the relationship between weight and mood. An analysis of 17 cross-sectional studies found that people who were obese were more likely to have depression than people with healthy weights. (17) Since the studies included in the analysis assessed weight and mood only at one point in time, the investigators could not say whether obesity increases the risk of depression or depression increases the risk of obesity. New evidence confirms that the relationship between obesity and depression may be a two-way street: A meta-analysis of 15 long-term studies that followed 58,000 participants for up to 28 years found that people who were obese at the start of the study had a 55 percent higher risk of developing depression by the end of the follow-up period, and people who had depression at the start of the study had a 58 percent higher risk of becoming obese. NCI supports research on obesity and cancer risk through a variety of activities, including large cooperative initiatives, web and data resources, extramural and intramural epidemiologic studies, basic science, and dissemination and implementation resources. For example, the Transdisciplinary Research on Energetics and Cancer (TREC) initiative links four research centers and a coordination center to investigate how the combined effects of obesity, poor diet, and low levels of physical activity increase cancer risk.
The wholesome foodies don’t argue that obesity and class are unrelated, but they frequently argue that the obesity gap between the classes has been created by the processed-food industry, which, in the past few decades, has preyed mostly on the less affluent masses. Yet Lenard Lesser, a physician and an obesity researcher at the Palo Alto Medical Foundation Research Institute, says that can’t be so, because the obesity gap predates the fast-food industry and the dietary dominance of processed food. “The difference in obesity rates in low- and high-income groups was evident as far back as we have data, at least back through the 1960s,” he told me. They are at increased health risk because of their fat distribution. While obesity of any kind is a health risk, it is better to be a pear than an apple.
Diagnosis of obesity is made by observation and by comparing the patient’s weight to ideal weight charts. Many doctors and obesity researchers refer to the body mass index (BMI), which uses a height-weight relationship to calculate an individual’s ideal weight and personal risk of developing obesity-related health problems. Physicians may also obtain direct measurements of an individual’s body fat content by using calipers to measure skin-fold thickness at the back of the upper arm and other sites. The most accurate means of measuring body fat content involves immersing a person in water and measuring relative displacement; however, this method is very impractical and is usually only used in scientific studies requiring very specific assessments. Women whose body fat exceeds 30% and men whose body fat exceeds 25% are generally considered obese.
Talk to your doctor if you notice weight gain while you are using one of these medicines. Ask if there are other forms of the same medicine or other medicines that can treat your medical condition, but have less of an effect on your weight. Do not stop taking the medicine without talking to your doctor.
The body mass index (BMI) is commonly used to determine whether someone is affected by excess weight or obesity. This is a measurement that is calculated using a person’s weight and height. As the BMI increases, the likelihood of being affected by obesity increases as well. Physicians are required to calculate and record this number in their patient’s chart. As a screening tool, it helps them identify weight issues that need to be addressed. Nevertheless, there are instances where this marker is not completely accurate. This can happen in the young and old alike but for different reasons. As it doesn’t differentiate between the type of excess body weight, it cannot determine if the excess weight consists of muscle or fat.

Certain medications may cause you to gain weight. These include some corticosteroids, antidepressants, and seizure medications. Medication can slow the rate at which your body burns calories, increase your appetite, or cause your body to hold extra water. All of these factors can lead to weight gain. If weight gain is a side effect of a medication you are taking, talk to your doctor about potentially switching to a different medication that isn’t associated with weight gain.

Even if Whole Foods, Real Food Daily, or the Farmhouse weren’t three bus rides away for the working poor, and even if three ounces of Vegan Cheesy Salad Booster, a Sea Cake appetizer, and the vegetarian quiche weren’t laden with fat and problem carbs, few among them would be likely to shell out $5.99, $9.95, or $16, respectively, for those pricey treats.
A constellation of conditions that place people at high risk for coronary artery disease. These conditions include type 2 diabetes, obesity, high blood pressure, and a poor lipid profile with elevated LDL (“bad”) cholesterol, low HDL (“good”) cholesterol, elevated triglycerides. Too much weight is especially hazardous for an aging body. Obesity exacerbates bone and muscle loss, increases inflammation and significantly raises the risk of diabetes, heart disease and stroke. Excess weight also increases the risk of developing chronic diseases, losing the ability to walk or dying earlier.

She will track the weight and overall health of the infants in her study, comparing them with C-section babies who did not receive the gauze treatment.
Moderate intensity aerobic exercise, 30 minutes a day, five times per week is currently recommended for adults ages 65 and older, according to the guidelines presented by the American College Sports of Medicine (ACSM). Those who are not used to exercising can start out with a shorter duration at a lower intensity and work up to the recommendations.
Next, ask the owner about specific body systems and other clinical signs. Cats with abdominal pain may lie in an unusual position or object to being held in a way that puts pressure on the abdomen. Vomiting and diarrhea may help localize the problem to the GI tract, although these are nonspecific signs of many conditions.
In the long term, a child with obesity is more likely to have obesity as an adult.24An adult with obesity has a higher risk of developing heart disease, type 2 diabetes, metabolic syndrome, and many types of cancer.25
For obese or overweight people who have two or more risk factors, federal guidelines recommend weight loss. Even a small amount of weight loss (such as ten percent of your current weight) lowers your chance of developing diseases associated with obesity. Patients who are overweight but have less than two risk factors and do not have a high waist measurement may just need to prevent further weight gain rather than lose weight.
Obesity-related illnesses now cost $147 billion each year, according to research from the Centers for Disease Control and Prevention (CDC). According to the CDC, medical costs associated with obesity increased from 6.5 percent of all medical spending in 1998 to about 9 percent in 2006.

A good night’s sleep: Deep rest helps the entire body function properly. Sleep modulates neuroendocrine function and glucose metabolism. Poor quality sleep can result in metabolic alterations such as glucose intolerance and a variation in the appetite-regulating hormones.
In the second paper, all CVD risk factors significantly improved in the diet and exercise group (Villareal 2006b). Specific mechanisms were not proposed, but the discussion focused on medical care costs related to metabolic coronary heart disease (CHD) risk factors that were ameliorated by the intervention (Table 1). In the third paper (Villareal 2008), bone turnover was measured by type 1 collagen C-terminal telopeptide (CTX), osteocalcin, and bone-specific alkaline phosphatase. There was a marked increase in serum CTX (~100-fold) and osteocalcin (~60-fold) concentrations in response to weight loss indicating that bone resorption and formation, respectively, were stimulated. Moreover, the increases in both CTX and osteocalcin concentrations correlated with decreases in hip bone mineral density (BMD), suggesting that weight-loss induced bone loss was due to increased bone turnover, with greater stimulation of bone resorption than bone formation. However, the clinical significance of the decrease in BMD was not clear as all participants had high baseline BMD Z-scores, and none had evidence of osteoporosis following weight loss. The investigators argued that BMD was not lost in the spine, which implies that the exercises were more effective in preserving BMD at this site. Exact mechanisms for loss of BMD with weight loss are not currently elucidated, but it was suggested that weight loss decreases the mechanical stress on the hip, without negatively impacting the spine or wrist. Weight loss was also associated with a 25% reduction in serum leptin that was highly correlated with decreased hip BMD. No such relationship was found between decreasing estradiol and changes in BMD. Leptin was discussed in the context of its inhibiting action on the expression of receptor activator of nuclear factor κB (NF-κB) ligand levels (Burguera 2001) and osteoblast differentiation (Cornish 2002). Levels of insulin-like growth factor 1 (IGF-1), cortisol, and parathyroid hormone (PTH) did not change in response to weight loss, which suggests that these bone-active hormones were not involved with the loss of BMD in the hip. Vitamin D supplementation during the trial did not reach optimal serum concentrations and whether higher dose Vitamin D supplementation could have slowed bone loss, was raised by the investigators. It was also noted that bone quality was not measured and could have been positively impacted by the exercise training intervention.

According to the CDC, the prevalence of obesity in the United States differs among racial/ethnic groups. For example, in 2011–2012 among adults, non-Hispanic blacks had the highest prevalence of obesity (47.8%) followed by Hispanics (42.0%), non-Hispanic whites (33.4%), and non-Hispanic Asians (10.9%) (5). Body mass index (BMI) is used to determine if you or your child are underweight, healthy, or overweight or obese. If both ate a calorie-restricted diet of 1,200 calories per day, the heavier person would lose weight faster. Age also is a factor in calorie expenditure. Metabolic rate tends to slow as we age, so the older a person is, the harder it is to lose weight.
The weight-loss program should be directed toward a slow, steady weight loss unless your doctor feels your health condition would benefit from more rapid weight loss. In patients over 65, the increase in chronic diseases associated with aging reduces physical activity and exercise capacity, making it more difficult for elderly persons to lose weight. The large number of older people with obesity and associated serious health risks make understanding the causes of obesity crucial. Obese older adults are more likely to be severely disabled and require the assistance of another person than those who are not obese (Center on an Aging Society, 2003). Older adults who are obese are more likely to suffer from persistent and chronic symptoms of illness, and to report symptoms of depression. In addition to having difficulty with activities of daily living, older obese adults are more likely to not be able to walk very far, go shopping, or participate in other activities that enrich our lives (Center on an Aging Society).
One of the two major types of diabetes, the type in which the beta cells of the pancreas produce insulin but the body is unable to use it effectively because the cells of the body are resistant to the action of insulin. Although this type of diabetes may not carry the same risk of death from ketoacidosis, it otherwise involves many of the same risks of complications as does type 1 diabetes (in which there is a lack of insulin).

Excess weight impairs respiratory function via mechanical and metabolic pathways. The accumulation of abdominal fat, for example, may limit the descent of the diaphragm, and in turn, lung expansion, while the accumulation of visceral fat can reduce the flexibility of the chest wall, sap respiratory muscle strength, and narrow airways in the lungs. While this research paradox carries on, obesity in our elders can’t be ignored.  Being overweight or obese can be exacerbated in some people after retirement because they have more free time to sit around and aren’t as active as when they were younger or worked. On the other hand, baby boomers are being forced to delay retirement because of the economy. So many of those people are at desk jobs all day instead of being able to retire and be active.

Mirtazapine (Remeron), a serotonin antagonist used to treat depression, has gained interest as a possible treatment for unintentional weight loss in older patients because 12% of patients who take this drug for depression report weight gain.36 Although no literature exists to support its use for unintentional weight loss, mirtazapine may be an option for older patients with depression who also have unintentional weight loss. Because dizziness and orthostatic hypotension are possible adverse effects of mirtazapine, caution is warranted in patients at risk of falls.36,37

An initial step in detecting liver damage is a simple blood test to determine the presence of certain liver enzymes in the blood. Under normal circumstances, these enzymes reside within the cells of the liver. But when the liver is injured, these enzymes are spilled into the blood stream, and can lead to diseases like fatty liver, type 2 diabetes, obesity, and hepatitis. Several medications also can increase liver enzyme test results.
The widespread availability of nutritional guidelines[90] has done little to address the problems of overeating and poor dietary choice.[91] From 1971 to 2000, obesity rates in the United States increased from 14.5% to 30.9%.[92] During the same period, an increase occurred in the average amount of food energy consumed. For women, the average increase was 335 calories (1,400 kJ) per day (1,542 calories (6,450 kJ) in 1971 and 1,877 calories (7,850 kJ) in 2004), while for men the average increase was 168 calories (700 kJ) per day (2,450 calories (10,300 kJ) in 1971 and 2,618 calories (10,950 kJ) in 2004). Most of this extra food energy came from an increase in carbohydrate consumption rather than fat consumption.[93] The primary sources of these extra carbohydrates are sweetened beverages, which now account for almost 25 percent of daily food energy in young adults in America,[94] and potato chips.[95] Consumption of sweetened drinks such as soft drinks, fruit drinks, iced tea, and energy and vitamin water drinks is believed to be contributing to the rising rates of obesity[96][97] and to an increased risk of metabolic syndrome and type 2 diabetes.[98] Vitamin D deficiency is related to diseases associated with obesity.[99]

It’s important for your senior to maintain body weight once they are advanced in age, experiencing frailty, or undergoing treatments for cancer. Ephedra: This natural substance is essentially an herbal phen-fen. It is the active ingredient in MaHuang and is used as a stimulant and appetite suppressant. Ephedra resembles the amphetamines — the popular “diet drugs” that were banned in the 1970s — in that it is highly addictive. Ephedra is often combined with caffeine and aspirin (“the Stack”), which increases the thermogenic (fat-burning) effect of ephedra. Ephedra increases the risk of high blood pressure, irregular heartbeat, insomnia, seizures, heart attack, stroke, and death. The FDA has recently banned ephedra because it has been linked to more than 100 deaths.
Results from this carefully designed study show the “diet-exercise group” preserved more lean muscle and bone density when compared to the other groups. They also gained significantly better physical function and were less frail than other groups, outperforming in all measured parameters. (See Figure A: Results of Physical Performance Test (PPT).)

For Amy Campbell, a registered dietitian and diabetes educator, the DASH, TLC and Mediterranean diets stood out as smart choices for older adults, because they’re good for weight loss as well as controlling conditions such as diabetes and high blood pressure.
Hypothyroidism. People with this condition have low levels of thyroid hormones . These low levels are associated with decreased metabolism  and weight gain, even when food intake is reduced. People with hypothyroidism also produce less body heat, have a lower body temperature, and do not efficiently use stored fat for energy.
Women in the baby boomer generation are somewhat more likely than men of the same age to feel that they are overweight, and less likely to feel that their weight is “about right.” Fifty-nine percent of baby boomer women say they are overweight, compared with 48% of baby boomer men. If the most-influential voices in our food culture today get their way, we will achieve a genuine food revolution. Too bad it would be one tailored to the dubious health fantasies of a small, elite minority. And too bad it would largely exclude the obese masses, who would continue to sicken and die early. Despite the best efforts of a small army of wholesome-food heroes, there is no reasonable scenario under which these foods could become cheap and plentiful enough to serve as the core diet for most of the obese population—even in the unlikely case that your typical junk-food eater would be willing and able to break lifelong habits to embrace kale and yellow beets. And many of the dishes glorified by the wholesome-food movement are, in any case, as caloric and obesogenic as anything served in a Burger King.
Still, plenty of older adults can benefit from losing weight, particularly if they’re obese, have weight-related chronic conditions or a poor quality of life. “If a person is overweight, they [often] feel better if they’ve lost weight,” says Dr. James Powers, a geriatrician and professor in the Vanderbilt University School of Medicine. “There’s less wear on the joints, greater endurance, greater ability to walk; to do normal activities without getting short of breath.”
One new study found that baby boomers (ages 49 to 67 in 2013) are living longer than people roughly 20 years older, but are not healthier.1 While they are less likely to smoke, have emphysema, or a heart attack, they are more likely to be obese, have diabetes, or high blood pressure than the previous generation at similar ages.
A team from the University of (UB) led by Dr Juan Carlos Laguna published a study in the journal Hepatology that provides clues to the molecular mechanism through which fructose (a type of sugar) in beverages may alter lipid energy metabolism and cause fatty liver and metabolic syndrome.
Several medications to stimulate appetite are available, but none have been shown to reduce mortality in older patients with unintentional weight loss.17,27 Megestrol (Megace), the most commonly studied medication, has been shown to improve appetite and increase weight gain in patients with cancer and AIDS cachexia35; however, studies in older patients are limited, and there are insufficient data to define an optimal dose.27,35 Adverse effects of megestrol include gastrointestinal upset, insomnia, impotence, hypertension, thromboembolic events, and adrenal insufficiency. Therefore, megestrol may not be appropriate for all patients, and the risks vs. benefits and patient preferences should be considered.18

My 46 year old dad has been over weight for about 11 years now due to excessive eating. He does play an older leage softball team and bowling but this has caused him pain in his fingers. He is trying to find a way to lose weight but without hurting his left knee which pops out at random times and is uncomfortable for him.

“In older, obese people, it may be more important to improve physical function and quality of life, rather than to reverse or treat risk factors for cardiovascular disease,” says Villareal, now chief of geriatrics at the New Mexico Veterans Affairs Health Care System and professor of medicine at the University of New Mexico School of Medicine, both in Albuquerque. “Combining exercise and weight loss isn’t designed so much to extend their life expectancy as it is to improve their quality of life during their remaining years and to help seniors avoid being admitted to a nursing home.”
Obesity has also been clearly linked to a lesser overall quality of life, which is of particular concern to the aging adult. Seniors can already be plagued by multiple conditions that decrease their quality of life, and obesity only adds another burden.
Commonly referred to as GERD or acid reflux, is a condition in which the liquid content of the stomach regurgitates (backs up or refluxes) into the esophagus. The liquid can inflame and damage the lining (cause esophagitis) of the esophagus although visible signs of inflammation occur in a minority of patients.
In this study, Villareal and his colleagues evaluated the effects of dieting and exercise in more than 100 obese seniors over a one-year period. Although weight loss alone and exercise alone improved physical function by about 12 percent and 15 percent, respectively, neither was as effective as diet and exercise together, which improved physical performance by 21 percent.
The NCI study included half a million members of the AARP, the nation’s largest group representing older Americans. Participants were between the ages of 50 and 71 at enrollment, and were followed for 10 years.
Weight-loss teas contain strong botanical laxatives (Senna, cascara sagrada) and diuretics (Rhamnus purshiana) that cause diarrhea and loss of water from the body. Diarrhea and water loss lead to the depletion of sodium and potassium and can lead to dehydration. Although an individual’s weight may decrease, the loss is due to a decrease in fluid and is only temporary. Moreover, low sodium and potassium levels may cause abnormal heart rhythms and can even lead to death.
Of course, if you don’t have a consistent weight training regimen, you’ll want to start slowly and lift light weights; this will give your body time to adapt without placing too much strain on your muscles or joints and help you avoid injury, says Huizenga. However, don’t get too comfortable with an easy resistance-training program. It is important to aim to gradually increase the amount of weight you lift. “It’s critical that significant resistance exercise be incorporated into any fat loss plan over age 60.” Once you can do 10 to 12 reps with, say, a 5-pound dumbbell and feel like you could keep going, it’s time to upgrade to an 8-pound weight, and so forth. “You know you’re lifting the right amount of weight if you can just barely make it to the end of your repetitions before needing to rest,” he says.
Factors that may contribute to weight gain among adults and youth include genes, eating habits, physical inactivity, TV, computer, phone, and other screen time, sleep habits, medical conditions or medications, and where and how people live, including their access to healthy foods and safe places to be active.1,6

Their increased health care cost will push America into a debt crisis. One driver will be paying for extended medical care to medicate the life choices made by the boomer generation. The second driver will be the increased gross amounts the boomer generation will be paid from Social Security by living longer.
Liposuction is a surgical procedure that removes excess fat that is difficult to get rid of with diet and exercise. Liposuction does not prevent weight regain. Liposuction has a good safety record; risks of the procedure include infection and skin discoloration.

What and how much a person eats. This aspect may involve keeping a food diary and developing a better understanding of the nutritional value and fat content of foods. It may also involve changing grocery-shopping habits (e.g., buying only what is on a prepared list and only going on a certain day), timing of meals (to prevent feelings of hunger, a person may plan frequent, small meals), and actually slowing down the rate at which a person eats.
The next class (category) of drugs changes the metabolism of fat. Orlistat (Xenical, alli) is the only drug of this category that is U.S. FDA approved. This is a class of anti-obesity drugs called lipase inhibitors, or fat blockers. Fat from food can only be absorbed into the body after being broken up (a process called digestion) by digestive enzymes called lipases in the intestines. By inhibiting the action of lipase enzymes, orlistat prevents the intestinal absorption of fat by 30%. Drugs in this class do not affect brain chemistry. Theoretically, orlistat also should have minimal or no systemic side effects (side effects in other parts of the body) because the major locale of action is inside the gut lumen and very little of the drug is absorbed.
Cancer. is also linked to higher rates of certain types of cancer (NIH, 2006). Breast cancer in older women is increasingly being linked to obesity (Sweeney, Blair, Anderson, Lazovich, & Folsom, 2004). Twenty-five to 30% of several major cancers, including breast (postmenopausal), colon, kidney, and esophageal, have been linked to obesity and physical inactivity (Vainio & Bianchini, 2002). Men who are obese are more likely to develop cancer of the colon, rectum, or prostate, than men who are not obese. Cancer of the gallbladder, uterus, cervix, or ovaries are more common in women who are obese compared with women who are not obese (NIH, 2006). Management of obesity is needed to decrease the incidence of these cancers.
Although the prevalence of obesity in persons who are over 80 years of age  is about one-half that of older adults between the ages of 50 and 59, the fact is that more than 15% of the older American population is obese (Villareal et al., 2005). Moreover, as the aging population increases in number, so too will the number of chronic illnesses, which often accompany aging, increase in our society (Flood & Newman, 2007). Chronic conditions, such as arthritis, diabetes, hypertension, and heart disease, are among some of the most common, debilitating, and costly chronic conditions in older adults. These conditions are frequently accentuated by obesity (Federal Interagency Forum on Aging, 2006).
According to a report by Claire Wang — researcher at the Department of Health Policy and Management at Columbia University’s Mailman School of Public Health — and her colleagues, the rising burden of obesity among people 60 and older is intensifying. Of the 65 million additional obese people projected in the U.S. in 2030, 24 million would be in this age range.

Insulin resistance. Insulin is necessary for the transport of blood glucose (sugar) into the cells of muscle and fat (which is then used for energy). By transporting glucose into cells, insulin keeps the blood glucose levels in the normal range. Insulin resistance (IR) is the condition whereby the effectiveness of insulin in transporting glucose (sugar) into cells is diminished. Fat cells are more insulin resistant than muscle cells; therefore, one important cause of insulin resistance is obesity. The pancreas initially responds to insulin resistance by producing more insulin. As long as the pancreas can produce enough insulin to overcome this resistance, blood glucose levels remain normal. This insulin resistance state (characterized by normal blood glucose levels and high insulin levels) can last for years. Once the pancreas can no longer keep up with producing high levels of insulin, blood glucose levels begin to rise, resulting in type 2 diabetes, thus insulin resistance is a pre-diabetes condition.

Examples of nutrient dense foods include sliced fruits and cooked vegetables, dairy products, and fish, chicken, and other lean proteins that are easy to chew and swallow. Sometimes, softer foods such as pudding, yogurt, or applesauce are helpful for increasing senior nutrition, and filling in calorie gaps in older adults.

Assessment should establish the cause, and, if reversible, treat accordingly. When patients state their weight loss is the result of dieting, probe for lifestyle changes. Maintaining weight loss is difficult, and if the patient is keeping the pounds off easily, dieting may be a coincidental occurrence.
Your exercise can be done all at one time or intermittently over the day. Initial activities may be walking or swimming at a slow pace. Your regimen can be adapted to other forms of physical activity, but walking is a particularly smart choice because of its safety and accessibility. Increase activity by undertaking frequent, less strenuous exercises, such as walking up and down the stairs instead of the using the elevator. You may eventually be able to engage in more strenuous activities such as tennis or any form of group sport.
Several randomized clinical trials in breast cancer survivors have reported weight loss interventions that resulted in both weight loss and beneficial changes in biomarkers that have been linked to the association between obesity and prognosis (43, 44). The “obesity paradox” refers to the unexpected findings that obese subjects seem to fare better than, or at least as well as, their normal- or low-weight counterparts in terms of mortality rates in the context of conditions, such as coronary artery disease in hypertensive subjects, congestive heart failure, chronic kidney disease, hemodialysis, postcoronary revascularization, and some instances of non-ST segment elevation in myocardial infarction (12,13). It remains uncertain how older age interacts with these protective effects of excess adiposity. Additionally, obesity is not a general “savior” in acute medical conditions. Hence, this interesting and potentially critical phenomenon remains presently enigmatic, requiring case- and age-specific in-depth examination. As an example of this effect, some 20 years ago, obesity in the elderly was actually linked to the twofold increase of postmyocardial infarction and in-hospital mortality in subjects >65 years (14). Has the obese phenotype been changed by the environment, or chronic medical therapy, or rather, have advances in the quality of critical care preferentially affected the obese?
This review excluded 11% of the retrieved references because they were not written in English. The overall quality of the literature retrieved was low. As a result, there is insufficient evidence to support the routine use of either nutritional interventions or pharmacologic treatments to help address unintentional weight loss. High-quality, adequately powered trials are needed to determine what interventions are effective at addressing unintentional weight loss and the associated clinical outcomes, such as mortality.

In the second paper, all CVD risk factors significantly improved in the diet and exercise group (Villareal 2006b). Specific mechanisms were not proposed, but the discussion focused on medical care costs related to metabolic coronary heart disease (CHD) risk factors that were ameliorated by the intervention (Table 1). In the third paper (Villareal 2008), bone turnover was measured by type 1 collagen C-terminal telopeptide (CTX), osteocalcin, and bone-specific alkaline phosphatase. There was a marked increase in serum CTX (~100-fold) and osteocalcin (~60-fold) concentrations in response to weight loss indicating that bone resorption and formation, respectively, were stimulated. Moreover, the increases in both CTX and osteocalcin concentrations correlated with decreases in hip bone mineral density (BMD), suggesting that weight-loss induced bone loss was due to increased bone turnover, with greater stimulation of bone resorption than bone formation. However, the clinical significance of the decrease in BMD was not clear as all participants had high baseline BMD Z-scores, and none had evidence of osteoporosis following weight loss. The investigators argued that BMD was not lost in the spine, which implies that the exercises were more effective in preserving BMD at this site. Exact mechanisms for loss of BMD with weight loss are not currently elucidated, but it was suggested that weight loss decreases the mechanical stress on the hip, without negatively impacting the spine or wrist. Weight loss was also associated with a 25% reduction in serum leptin that was highly correlated with decreased hip BMD. No such relationship was found between decreasing estradiol and changes in BMD. Leptin was discussed in the context of its inhibiting action on the expression of receptor activator of nuclear factor κB (NF-κB) ligand levels (Burguera 2001) and osteoblast differentiation (Cornish 2002). Levels of insulin-like growth factor 1 (IGF-1), cortisol, and parathyroid hormone (PTH) did not change in response to weight loss, which suggests that these bone-active hormones were not involved with the loss of BMD in the hip. Vitamin D supplementation during the trial did not reach optimal serum concentrations and whether higher dose Vitamin D supplementation could have slowed bone loss, was raised by the investigators. It was also noted that bone quality was not measured and could have been positively impacted by the exercise training intervention.
The Robert Wood Johnson Foundation, which focuses on the nation’s public health, recently published research showing that in two states – Arkansas and Louisiana – more than 40 percent of boomers are obese. So are 30 percent of boomers in another 41 states, as well as almost 24 percent of Californians in that age category.

When you’re obese, your overall quality of life may be diminished. You may not be able to do things you used to do, such as participating in enjoyable activities. You may avoid public places. Obese people may even encounter discrimination.
• Organic. Cancer is the leading cause, accounting for 24% to 38% of cases (weight loss may be the only symptom of tumor burden).5 Dementia results in marked taste and smell alterations, decreasing food’s desirability. Dysphasia often accompanies severe dementia, as do centrally mediated deficits in appetite control and satiety. Other organic causes may be gastrointestinal ([GI]; nausea, vomiting, early satiety, diarrhea), endocrine (hyperthyroidism, uncontrolled diabetes), Parkinson’s disease, and chronic illness (chronic obstructive pulmonary disease, congestive heart failure).4,5
The next step is to study each diet and see how practical it is to include in your current lifestyle and level of activity. Your choice of diet will need to be stuck to for the long term and it must be something that you are comfortable doing. It should fit with your personality.
A number of additional health outcomes have been linked to excess weight. These include the development of gallstones in men (40) and women, (41) as well as gout, (42, 43) chronic kidney disease, (44) and nonalcoholic fatty liver disease. (25,45)

You also need strength training activities at least two days a week, according to the Centers for Disease Control and Prevention. Strength training will you from losing muscle as you age. Get the full benefit of strength training by completing eight to 12 repetitions until it’s difficult to complete a repetition without getting help. Lifting weights, resistance bands, heavy gardening or yoga are examples of strength training activities for older women.
How did the most educated and wealthiest generation of Americans to date allow its collective health to fall by the wayside? The American lifestyle has largely shifted from active to sedentary and from community-oriented to socially isolating. Adults experience more stress in their hectic daily lives, which breeds depression and health problems, such as hypertension and high blood pressure. Obesity is a major public health problem and the leading nutritional disorder in the U.S. It is responsible for more than 280,000 deaths annually in this country. A widely accepted definition of obesity is body weight that is 20% or more in excess of ideal weight:height ratio according to actuarial tables. By this definition, 34% of adults in the U.S. are obese. The National Institutes of Health have defined obesity as a BMI of 30 kg/m2 or more, and overweight as a BMI between 25 and 30 kg/m2. By these criteria, two thirds of adults are either overweight or obese. There is strong evidence that the prevalence of obesity is increasing in both children and adults. Increases are particularly striking among African-Americans and Mexican-Americans. More than 80% of black women over the age of 40 are overweight, and 50% are obese. Among factors blamed for the steady increase in the prevalence of obesity are unhealthful eating practices (high-fat diet, overlarge portions) and the decline in physical activity associated with use of automobiles and public transportation instead of walking, labor-saving devices including computers, and passive forms of entertainment and recreation (television, computer games). Despite efforts of public health authorities to educate the public about the dangers of obesity, it is widely viewed as a cosmetic rather than a medical problem. Obesity is an independent risk factor for hypertension, hypercholesterolemia, Type 2 diabetes mellitus, myocardial certain malignancies (cancer of the colon, rectum, and prostate in men and of the breast, cervix, endometrium, and ovary in women), obstructive sleep apnea, hypoventilation syndrome, osteoarthritis and other orthopedic disorders, infertility, lower extremity venous stasis disease, gastroesophageal reflux disease, and urinary stress incontinence. Lesser degrees of obesity can constitute a significant health hazard in the presence of diabetes mellitus, hypertension, heart disease, or their associated risk factors. Body fat distribution in central (abdominal or male pattern, with an increased waist:hip ratio) versus peripheral (gluteal or female pattern) adipose tissue depots is associated with higher risks of many of these disorders. Obese people are more liable to injury, more difficult to examine by palpation and imaging techniques, and more likely to have unsuccessful outcomes and complications from surgical operations. Not least among the adverse effects of obesity are social stigmatization, poor self-image, and psychological stress. Weight reduction is associated with improvement in most of the health risks of obesity. All treatments for obesity (other than cosmetic surgical procedures in which subcutaneous fat is mechanically removed) require creation of an energy deficit by reducing caloric intake, increasing physical exercise, or both. Basic weight reduction programs involve consumption of a restricted-calorie, low-fat diet and performance of at least 30 minutes of endurance-type physical activity of at least moderate intensity on most and preferably all days of the week. Behavior modification therapy, hypnosis, anorexiant drugs (sympathomimetic agents, sibutramine), the lipase inhibitor orlistat, and surgical procedures to reduce gastric capacity or intestinal absorption of nutrients are useful in selected cases, but the emphasis should be on establishing permanent changes in lifestyle. Weight reduction is not recommended during pregnancy or in patients with osteoporosis, cholelithiasis, severe mental illness including anorexia nervosa, or terminal illness.

As with obesity in adults, many factors contribute to the rising rates of childhood obesity. Changing diet and decreasing physical activity are believed to be the two most important causes for the recent increase in the incidence of child obesity.[228] Because childhood obesity often persists into adulthood and is associated with numerous chronic illnesses, children who are obese are often tested for hypertension, diabetes, hyperlipidemia, and fatty liver.[81] Treatments used in children are primarily lifestyle interventions and behavioral techniques, although efforts to increase activity in children have had little success.[229] In the United States, medications are not FDA approved for use in this age group.[227] Multi-component behaviour change interventions that include changes to dietary and physical activity may reduce BMI in the short term in children aged 6 to 11 years, although the benefits are small and quality of evidence is low.[230]

Obesity is known to cause heart disease, stroke, diabetes and certain cancers. Developing any of these illnesses and diseases can have debilitating results, including loss of employment, inability to cover medical costs and care, and disability. Savings can be wiped out, causing tremendous financial hardships — at a time when unemployment is at an all time high. And, to compound the problem, many Americans are living without health insurance, which is a huge financial risk in and of itself.
The first goal of dieting is to stop further weight gain. The next goal is to establish realistic weight-loss goals. While the ideal weight corresponds to a BMI of 20-25, this is difficult to achieve for many people. Thus, success is higher when a goal is set to lose 10%-15% of baseline weight as opposed to 20%-30% or greater. It is also important to remember that any weight reduction in an obese person would result in health benefits.
Until the late 1980s, malnutrition was widespread among poor Chileans, especially children. Today, three-quarters of adults are overweight or obese, according to the country’s health ministry. Officials have been particularly alarmed by childhood obesity rates that are among the world’s highest, with over half of 6-year-old children overweight or obese.
2. Get free nutrition and weight loss counseling. To help the nation’s 13 million obese seniors, the Affordable Care Act included a new Medicare benefit offering face-to-face weight loss counseling in primary care doctors’ offices. The WHO defines an adult who has a BMI between 25 and 29.9 as overweight – an adult who has a BMI of 30 or higher is considered obese – a BMI below 18.5 is considered underweight, and between 18.5 to 24.9 a healthy weight .
In the past few years the number of older adults who are obese has doubled, with more than 15% of the older adult population now classified as obese. More specifically, it is estimated that the prevalence of obesity in adults ages 50 to 69 is 22.9%; for adults over age 70, the obesity rate is 15%.
In addition, the researchers evaluated peak oxygen consumption during exertion with treadmill walking. On that test, obese elderly people who both dieted and exercised improved 17 percent from their baseline. The diet-only group showed a 10 percent gain, and the exercise-only group improved about 8 percent.
BMI ranges for children and teens are defined so that they take into account normal differences in body fat between boys and girls and differences in body fat at various ages. However although BMI correlates with the amount of body fat, BMI does not directly measure body fat and some people, such as athletes, may have a BMI that identifies them as overweight even though they do not have excess body fat.
In 2004, the United Kingdom Royal College of Physicians, the Faculty of Public Health and the Royal College of Paediatrics and Child Health released the report “Storing up Problems”, which highlighted the growing problem of obesity in the UK.[159] The same year, the House of Commons Health Select Committee published its “most comprehensive inquiry […] ever undertaken” into the impact of obesity on health and society in the UK and possible approaches to the problem.[160] In 2006, the National Institute for Health and Clinical Excellence (NICE) issued a guideline on the diagnosis and management of obesity, as well as policy implications for non-healthcare organizations such as local councils.[161] A 2007 report produced by Derek Wanless for the King’s Fund warned that unless further action was taken, obesity had the capacity to cripple the National Health Service financially.[162]
Rucker, who is obese herself, says she doesn’t expect her older patients to lose a lot of weight. “I think you’ll see weight loss of 10 to 20 pounds, but whether you’re going to see people lose 50 to 100 pounds as they’re older, I doubt it,” says Rucker.
Nearly all of the evidence linking obesity to cancer risk comes from large cohort studies, a type of observational study. However, data from observational studies can be difficult to interpret and cannot definitively establish that obesity causes cancer. That is because obese or overweight people may differ from lean people in ways other than their body fat, and it is possible that these other differences—rather than their body fat—are what explains their different cancer risk.

Losing weight is difficult, and interventions that work in younger adults cannot be assumed to translate to older populations with co-morbidities, low muscle mass and frailty 2004). The appropriate treatment approach for obesity remains highly contentious due to the lack of evidenced-based data demonstrating that long-term weight loss is net beneficial or harmful in this age group. There is evidence that successful weight loss is possible in adults 65 years and older (Villareal 2006a; Villareal 2006b; Villareal 2008; Frimel 2008; Lambert 2008; Shah 2009; Villareal 2011a; Armamento-Villareal 2012; Shah 2011; Kelly 2011). However, weight-loss trials have reported losses of lean body mass and bone mineral density, in addition to fat mass (Villareal 2006a; Villareal 2006b; Villareal 2008; Frimel 2008; Lambert 2008; Shah 2009; Villareal 2011a; Armamento-Villareal 2012; Shah 2011; Kelly 2011; Bales 2008). These negative outcomes discourage many geriatricians from advising weight loss to their obese older patients (Heiat 2001; Rossner 2001; Sorensen 2003; Villareal 2005; Zamboni 2005; Rolland 2006; Morley 2010), despite improvements in body composition, physical function, metabolic and cardiovascular parameters that accompany weight loss (Forsythe 2008; Anandacoomarasamy 2009; Cheung 2012; Erteck 2012). Given these positive functional and metabolic outcomes, it is somewhat surprising that advising weight loss in obese older adults is still shunned in the medical community (Houston 2009; Sommers 2011). Compounding the confusion surrounding risks versus benefits from intentional weight loss is the lack of human studies to elucidate the mechanisms associated with the loss of muscle and bone. Also lacking are trials with adequate follow-up to assess the behaviors associated with long-term maintenance of weight loss and health outcomes related to sustained weight loss.

A third study examined the impact of physical training and nutrition.18 Fifty-eight older, community-dwelling individuals were randomized to one of the following four groups: a physical training program (involving aerobic, muscle strength and balance training), a nutritional intervention program (involving individually targeted advice and group sessions), a combination of the first two interventions, or a control group. At baseline and then again at 12 weeks, subjects were screened for physical performance. Intention-to-treat analysis showed a significant improvement in both training groups compared with the nutritional group. The nutritional interventions showed no significant improvement over the control group.

In many instances, determining waist circumference seems to be a valuable measurement that may give physicians guidance in weight matters for their patients. It is, and should be, used as an additional tool although this is not always standard of care at this time.
Meal replacements. These plans suggest that you replace one or two meals with their products — such as low-calorie shakes or meal bars — and eat healthy snacks and a healthy, balanced third meal that’s low in fat and calories. In the short term, this type of diet can help you lose weight. Keep in mind that these diets likely won’t teach you how to change your overall lifestyle, though, so you may have to keep this up if you want to keep your weight off.

Ephedra: This natural substance is essentially an herbal phen-fen. It is the active ingredient in MaHuang and is used as a stimulant and appetite suppressant. Ephedra resembles the amphetamines — the popular “diet drugs” that were banned in the 1970s — in that it is highly addictive. Ephedra is often combined with caffeine and aspirin (“the Stack”), which increases the thermogenic (fat-burning) effect of ephedra. Ephedra increases the risk of high blood pressure, irregular heartbeat, insomnia, seizures, heart attack, stroke, and death. The FDA has recently banned ephedra because it has been linked to more than 100 deaths.
Significant regulation of junk food may not go far, but we have other tools at our disposal to prod Big Food to intensify and speed up its efforts to cut fat and problem carbs in its offerings, particularly if we’re smart about it. Lenard Lesser points out that government and advocacy groups could start singling out particular restaurants and food products for praise or shaming—a more official version of “eat this, not that”—rather than sticking to a steady drumbeat of “processed food must go away.” Academia could do a much better job of producing and highlighting solid research into less obesogenic, high-mass-appeal foods, and could curtail its evidence-light anti-food-processing bias, so that the next generation of social and policy entrepreneurs might work to narrow the gap between the poor obese and the well-resourced healthy instead of inadvertently widening it. We can keep pushing our health-care system to provide more incentives and support to the obese for losing weight by making small, painless, but helpful changes in their behavior, such as switching from Whoppers to turkey burgers, from Egg McMuffins to Egg White Delights, or from blueberry crisp to fruit-and-yogurt parfaits.

While Americans are consuming more calories, they are not expending them with enough physical activity. Physical activity is an important element in modifying and shaping behaviors. First of all, set realistic objectives. The latest guidelines reflect the goal of rapidly losing weight in a short period of time. The current recommendation is to achieve a 5-10% reduction in body weight over a six-month period and every six months thereafter until you’ve reached your ideal body weight.
The good news is that losing a small amount of weight can reduce your chances of developing heart disease or a stroke. Losing 5%-10% of your weight is proven to lower your chance of developing heart disease.
Though sometimes downplayed or even ignored by elderly patients, sudden weight loss is a serious cause for concern says Dr. Barry Fabius, medical director of geriatrics at Holy Redeemer Health System in Philadelphia, Pennsylvania. “My radar goes up,” says Fabius. “I immediately wonder if I’m dealing with cancer.”
Unintentional weight loss is defined as an involuntary decline in body mass, usually occurring over a relatively short period of time. The condition is quite common among elderly adults, particularly those over 70 years of age. Seniors who experience this form of weight loss lose some of their ability to function independently, see a reduction in quality of life and have an increased risk of mortality. Unintentional weight loss can result from a variety of conditions, with physical, mental, emotional and social factors playing potential roles. A common form of short stature, achondroplasia (dwarfism) is a genetic condition causing a disorder of bone growth. Complications of achondroplasia that need monitoring include (this is not all inclusive) stenosis and compression of the spinal cord, a large opening under the skull, lordosis, kyphosis, spinal stenosis, hydrocephalus, middle ear infections, obesity, and dental crowning. Achondroplasia is caused by mutations of the FGFR3 gene.
According to the National Institute of Health, the percentage of those seniors entering nursing homes who are moderate and severely obese — with a body mass index of 35 or greater — has risen sharply, to nearly 25% in 2010 from 14.7% in 2000, according to a recent study, and many signs suggest the upward trend is continuing.
Of course, if you don’t have a consistent weight training regimen, you’ll want to start slowly and lift light weights; this will give your body time to adapt without placing too much strain on your muscles or joints and help you avoid injury, says Huizenga. However, don’t get too comfortable with an easy resistance-training program. It is important to aim to gradually increase the amount of weight you lift. “It’s critical that significant resistance exercise be incorporated into any fat loss plan over age 60.” Once you can do 10 to 12 reps with, say, a 5-pound dumbbell and feel like you could keep going, it’s time to upgrade to an 8-pound weight, and so forth. “You know you’re lifting the right amount of weight if you can just barely make it to the end of your repetitions before needing to rest,” he says.
Another aim of this review was to report on weight maintenance and long-term health outcomes to determine if weight loss can be maintained beyond one year. It was unexpected that only one small follow-up pilot study fit our inclusion criteria (Waters et al. 2013).
