Frimel et al. (2008) reported on a cohort of 30 community-living frail older adults. The participants were sedentary (≤ 2 exercise sessions per week); had stable medications and stable weight (± 2 kg over the past year); and met two out of three criteria for mild–moderate physical frailty. For individuals who are severely obese, dietary changes and behavior modification may be accompanied by surgery to reduce or bypass portions of the stomach or small intestine. Although obesity surgery is less risky as of 2003 because of recent innovations in equipment and surgical technique, it is still performed only on patients for whom other strategies have failed and whose obesity seriously threatens their health. Other surgical procedures are not recommended, including liposuction, a purely cosmetic procedure in which a suction device is used to remove fat from beneath the skin, and jaw wiring, which can damage gums and teeth and cause painful muscle spasms.

Sleep-wake cycles can contribute to obesity. NHLBI continues funding studies to understand how the body’s internal sleep-wake cycles influence sleep and eating behaviors in people who are obese. This may help discover new therapies.

Obesity and overweight have in the last decade become a global problem – according to the World Health Organization (WHO) back in 2005 approximately 1.6 billion adults over the of age 15+ were overweight, at least 400 million adults were obese and at least 20 million children under the age of 5 years were overweight.

The World Health Organization (2005) has noted that life-threatening illnesses related to obesity include cardiovascular disease; conditions associated with insulin resistance, such as type 2 diabetes; certain types of cancers, especially hormonally related and large-bowel cancer; and gallbladder disease. The next few sections will discuss these illnesses.
Physical illnesses may also have secondary characteristics that lead to weight loss. For example, gall stones often result in decreased appetite and nausea, which causes them to avoid high-energy foods.
Excess weight impairs respiratory function via mechanical and metabolic pathways. The accumulation of abdominal fat, for example, may limit the descent of the diaphragm, and in turn, lung expansion, while the accumulation of visceral fat can reduce the flexibility of the chest wall, sap respiratory muscle strength, and narrow airways in the lungs. (32) Cytokines generated by the low-grade inflammatory state that accompanies obesity may also impede lung function.
Vagal nerve blockade is another treatment for obesity. It involves implanting a device under the skin of the abdomen that sends intermittent electrical pulses to the abdominal vagus nerve, which tells the brain when the stomach feels empty or full. This new technology received FDA approval in 2014 for use by adults who have not been able to lose weight with a weight-loss program and who have a BMI of 35 to 45 with at least one obesity-related condition, such as type 2 diabetes.
The food industry calls the rules government overreach. Felipe Lira, the director of Chilealimentos, an industry association, said the new nutrition labels were confusing and “invasive,” and that the marketing restrictions were based on a scientifically flawed correlation between the promotion of unhealthy foods and weight gain. “We believe that the best way to approach the problem of obesity is through consumer education that changes people’s habits,” he said in an emailed statement.
“We’re all creatures of habit,” Campbell says. So, she says, imagine you’re 75 years old and have to change your habits and incorporate new foods like tofu. Although most diets offer plenty of online and printed resources, they can be overwhelming. “It’s hard sometimes to pick up a book and say, ‘what should I be eating,'” she says. For older adults, it can help to work with dietitians.
Most seniors experience a steady decline in strength and endurance as they age, especially after age 75. Yoga routines for seniors help improve balance, strength and mobility, allowing for continued independence in later years as well as improving such characteristic health problems as arthritis, constipation and hypertension. Among other benefits, yoga practice promotes an increase in self-reported quality of life, as reported in a 2011 study published in the “International Journal of Yoga.”
Diabetes. Type 2 diabetes, the most common type of diabetes in older adults, results from interplay between genetic factors and environmental factors that contribute to obesity. Even a 15 pound weight gain can increase a person’s risk of diabetes by 50% (Daniels, 2006). There is an age-related increase in total body fat and visceral adiposity until age 65 that is often accompanied by diabetes or impaired glucose intolerance (Wilson & Kannel, 2007). In the Framingham Study 30-40% of people over 65 were found to have diabetes or glucose intolerance. Coronary disease is the most common and lethal sequel of type 2 diabetes. Lean-muscle mass begins to diminish after the age of 65. This decrease may be related to decreased physical activity, disability, anabolic hormone production, or increased cytokine activity. If calorie intake continues at the same rate while the muscle mass decreases, the older person will most likely experience fat weight gain (Tucker, 2006).
Pregnant women who are overweight are more likely to develop insulin resistance, high blood sugar, and high blood pressure. Overweight also increases the risks associated with surgery and anesthesia, and severe obesity increases surgery time and blood loss.
In elderly patients who suffer from serious health disease conditions such as cancer or heart disease, the amount of weight loss experienced. Unintentional weight loss may also point to an as of yet undiagnosed condition. Seniors who lose more than 4 percent of their body weight in a year or 10 percent over five years should seek medical attention to address the issue, as this degree of weight loss increases morbidity and mortality.
A chart review of 290 medical records from many centres in the United States that included long-term care residents and home care clients found six factors to be associated with unexplained weight loss.3 These factors included reduced functional ability, taking in 50% or less of the food served in three consecutive days, refusal of 50% or more of food replacement offered over a seven-day period, chewing problems, a serum albumin level less than 35 g/L with normal hydration status and a cholesterol level less than 4.1 mmol/L.
Another aim of this review was to report on weight maintenance and long-term health outcomes to determine if weight loss can be maintained beyond one year. It was unexpected that only one small follow-up pilot study fit our inclusion criteria (Waters et al. 2013).

The main ingredients in most herbal fen/phen products are ephedrine and St. John’s wort. Ephedrine acts like amphetamines in stimulating the central nervous system and the heart. Ephedrine promotes weight loss in part by an increase the body’s temperature, and when this happens, the body burns more calories. Ephedrine use has been associated with high blood pressure, heart-rhythm irregularities, strokes, insomnia, seizures tremors, and nervousness. There have been reports of deaths in young individuals taking ephedrine. St. John’s wort has been used in Europe to treat mild depression but not obesity. The action, effectiveness, and side effects of St. John’s wort either alone or in combination with other agents have not been adequately studied.
The points in this article clearly show that in the elderly population, weight classification may not always be accurate, which is one of the difficulties encountered in older adults. This is the first hurdle encountered when trying to evaluate and treat this disease in the elderly.
Lack of access to healthy foods. Some people don’t live in neighborhoods with supermarkets that sell healthy foods, such as fresh fruits and vegetables. Or, for some people, these healthy foods are too costly.
Fortunately, there are lots of weight-loss options available for boomers, and all the good ones revolve around an age-old formula for shedding pounds: eating less and exercising more. That’s something any generation can get behind.
Medical problems. In people, obesity can be traced to a medical cause, such as Prader-Willi syndrome, Cushing’s syndrome and other conditions. Medical problems, such as arthritis, also can lead to decreased activity, which may result in weight gain.
In some cases, weight-loss surgery, also called bariatric surgery, is an option. Weight-loss surgery limits the amount of food you’re able to comfortably eat or decreases the absorption of food and calories or both. While weight-loss surgery offers the best chance of losing the most weight, it can pose serious risks.
Keep in mind, though, that weight-loss medication is meant to be used along with diet, exercise and behavior changes, not instead of them. If you don’t make these other changes in your life, medication is unlikely to work.
Documenting such differences does not mean the discrepancies are responsible for obesity, however. To demonstrate cause and effect, Gordon and his colleagues conducted an elegant series of experiments with so-called humanized mice, published last September in Science. First, they raised genetically identical baby rodents in a germ-free environment so that their bodies would be free of any bacteria. Then they populated their guts with intestinal microbes collected from obese women and their lean twin sisters (three pairs of fraternal female twins and one set of identical twins were used in the studies). The mice ate the same diet in equal amounts, yet the animals that received bacteria from an obese twin grew heavier and had more body fat than mice with microbes from a thin twin. As expected, the fat mice also had a less diverse community of microbes in the gut.
Lean body mass declines at a rate of 0.3 kg/year (0.66 lb) beginning around the third decade of life. Because lean body mass tends to be replaced by fat, total body weight generally remains stable. Beginning around age 65 to 70, weight loss occurs at a rate of 0.1 to 0.2 kg/year as a result of changes in hormones regulating appetite and satiety, along with decreases in basal metabolic rates.6,8
Asthma is a condition in which hyperreactive airways constrict and result in symptoms like wheezing, coughing, and shortness of breath. Causes of asthma include genetics, environmental factors, personal history of allergies, and other factors. Asthma is diagnosed by a physician based on a patient’s family history and results from lung function tests and other exams. Inhaled corticosteroids (ICS) and long-acting bronchodilators (LABAs) are used in the treatment of asthma. Generally, the prognosis for a patient with asthma is good. Exposure to allergens found on farms may protect against asthma symptoms.
Obesity is mostly preventable through a combination of social changes and personal choices.[1] Changes to diet and exercising are the main treatments.[2] Diet quality can be improved by reducing the consumption of energy-dense foods, such as those high in fat and sugars, and by increasing the intake of dietary fiber.[1] Medications may be used, along with a suitable diet, to reduce appetite or decrease fat absorption.[5] If diet, exercise, and medication are not effective, a gastric balloon or surgery may be performed to reduce stomach volume or length of the intestines, leading to feeling full earlier or a reduced ability to absorb nutrients from food.[6][12]

“Telling it like it is,” was sports journalist Howard Cosell’s mantra, which he preached to the baby boomer generation that joined him in making Monday Night Football a national party night. In the spirit of “telling it like it is,” this is what the numbers now say say about the baby boomer generation: Medicare (which baby boomers are signing up for in record numbers to avoid soaring health care costs) is projected to spend 72 percent more for the remaining lifetime of a typical 65-year-old in 2030 than a 65-year-old in 2010. Obesity is a major reason why. Almost twice as many baby boomers will suffer from obesity in 2030 compared to 2010.
For the better part of 71 years, Joe Acosta ate what he wanted or what his wife prepared for him – often, large portions of fried foods. As a result, he carried 30-some pounds of extra weight and flirted with diabetes. “I was having problems watching what I put in my body,” says Acosta, a retired U.S. Postal Service employee in Brooklyn, New York.
The good news is that even modest weight loss can improve or prevent the health problems associated with obesity. Dietary changes, increased physical activity and behavior changes can help you lose weight. Prescription medications and weight-loss surgery are additional options for treating obesity.

After my excursion to Whole Foods, I drive a few minutes to a Trader Joe’s, also known for an emphasis on wholesome foods. Here at the register I’m confronted with a large display of a snack food called “Inner Peas,” consisting of peas that are breaded in cornmeal and rice flour, fried in sunflower oil, and then sprinkled with salt. By weight, the snack has six times as much fat as it does protein, along with loads of carbohydrates. I can’t recall ever seeing anything at any fast-food restaurant that represents as big an obesogenic crime against the vegetable kingdom. (A spokesperson for Trader Joe’s said the company does not consider itself a “ ‘wholesome food’ grocery retailer.” Living Intentions did not respond to a request for comment.)

The third paper, Shah et al. (2011), focused on bone loss measuring C-terminal telopeptide of type I collagen (CTX) as a marker of bone resorption, and osteocalcin and N-terminal propeptide of type I procollagen as a markers of bone formation. Bone-active hormones, serum estradiol, IGF-1, 25-hydroxyvitamin D, and serum PTH concentration were also obtained. Serum C-terminal telopeptide (CTX) and osteocalcin increased in the diet group, with bone resorption increasing more than bone formation. Both of these markers decreased in the exercise and control groups. Osteocalcin did not change with the combination of diet and exercise. Serum leptin and estradiol concentrations decreased more markedly in diet plus exercise than in diet alone (−38% and −13%, respectively). It was suggested that the decrease in leptin with weight loss could stimulate the receptor activator of NF-κB ligand and the receptor activator of NF-κB pathway leading to increased bone resorption and bone loss (Burguera 2001; Thomas 2002). There was no decrease in IGF-1 with weight loss and this was attributed to adequate protein intake during weight loss. Change in mechanical stress was cited as the mechanism behind BMD loss in the hip, but not in the spine or whole body. The most important finding was that in these obese older adults supplemented with calcium and vitamin D, exercise training added to weight loss offset increased bone turnover and loss of BMD. This was supported by changes in lean body mass, 1RM strength and osteocalcin, the only variables that remained in the final regression model predicting the changes in hip BMD, suggesting that exercise countered the unloading effect of weight loss on BMD.
Diabetes: About 9 percent of adults worldwide have diabetes, according to the World Health Organization (WHO). In the U.S., more than 30 million people have diabetes and more than 86 million have pre-diabetes, according to the American Diabetes Association.
Obesity increases the risk of diabetes and high blood pressure, the most common causes of chronic kidney disease. Recent studies suggest that even in the absence of these risks, obesity itself may promote chronic kidney disease and quicken its progress.
If your doctor says you’re overweight, that means “you’re slightly over what’s considered healthy,” says Y. Claire Wang, MD. She’s co-director of the Obesity Prevention Initiative at Columbia University.
They found that the “normal” body weight of mice that become obese starts going up; their bodies’ perception of normal weight becomes a heavier than before, regardless of whether they are made to go on diets which had made them lose weight.
Children in low- and middle-income countries are more vulnerable to inadequate pre-natal, infant, and young child nutrition. At the same time, these children are exposed to high-fat, high-sugar, high-salt, energy-dense, and micronutrient-poor foods, which tend to be lower in cost but also lower in nutrient quality. These dietary patterns, in conjunction with lower levels of physical activity, result in sharp increases in childhood obesity while undernutrition issues remain unsolved.

Given that many older adults suffer from obesity, you will likely have some of these clients coming to you for help in losing fat and increasing muscle mass and strength. Of course, strength training along with sensible eating can be instrumental in bringing about desired changes in overall body composition. Using the workouts in chapter 4, you can easily adjust training loads or resistances to match current strength levels while selecting exercises that can be performed safely on sturdy and properly sized machine and free-weight equipment.
In 1850, the average man had a normal body mass index (BMI) of 23. Fast forward to 2000, and the male frame elongated and ballooned to a BMI of 28.2, teetering on the brink of obesity [source: Kolata]. According to the Centers for Disease Control and Prevention (CDC), adults aged 40 to 59 — aka baby boomers — have the highest prevalence of obesity. Of that group, 40 percent of men and 41 percent of women were obese in 2007 [source: CDC]. Their parents, however, had a lower overall obesity rate.
Increased body mass has a negative impact on weight-bearing joints, and knee osteoarthritis is particularly common in obese men (58%) and women (68%) by the age of ≥65 years (41). PRB observes that baby boomers are living longer than previous generations. They also have, on average, higher levels of education and more work experience, which bode well for their economic security in old age.
Even if there’s nothing wrong with your health it’s quite common for older people to lose their appetite. You may be underweight simply because you’re not eating enough and your diet doesn’t give you sufficient energy or calories.
Finally, cumulative attrition of the most vulnerable fraction of the obese population brought about by premature mortality of those subjects who do not survive the late-midlife years leaves only the most biologically advantaged obese survivors for “nonbiased” epidemiological analysis of obesity in advanced years. If one accepts that obesity increases mortality in younger years, attempted comparison between age-matched obese and lean humans in the older age inevitably leads to the study of two highly unequal cohorts of which only one has been subjected to the Darwinian process of obesity-related attrition.

As women get older, belly fat is a common problem. Consuming monounsaturated fats can combat this problem by increasing your basal metabolic rate. Foods rich in this type of fat include avocados, almonds and peanuts. Vegetable-based oils, like canola oil and olive oil, are a few other options. Supplement high-fat foods, like butter, with these healthy options for increased fat loss.
Body Mass Index (BMI) is a mathematical calculation involving height and weight, irrespective of family history, gender, age or race. BMI is calculated by dividing a person’s body weight in kilograms by their height in meters squared (weight [kg] height [m]2) or by using the conversion with pounds (lbs) and inches (in) squared as shown below, This number can be  misleading, however, for very muscular people, or for pregnant or lactating women.
If you are considered to be overweight, losing as little as 5 percent of your body weight may lower your risk for several diseases, including heart disease and type 2 diabetes. If you weigh 200 pounds, this means losing 10 pounds. Slow and steady weight loss of 1/2 to 2 pounds per week, and not more than 3 pounds per week, is the safest way to lose weight.
Cancers of the colon, breast (after menopause), endometrium (the lining of the uterus), kidney, and esophagus are linked to obesity. Some studies have also reported links between obesity and cancers of the gallbladder, ovaries, and pancreas.
“With this new study, we will be able to discern whether a shift in disability is due to a change in physical or cognitive capacity or to changes in the accommodations people make,” explained Freedman. The accommodations measured include behavior changes (such as bathing less often), asking someone for help, and using assistive devices and home modifications (a bath seat, for example). Among the goals, she said, is to identify the ways people adapt to disabilities that allow them to remain independent as long as possible.
This phenomenon is by no means limited to packaged food at upscale supermarkets. Back in February, when I was at Real Food Daily in Los Angeles, I ordered the “Sea Cake” along with my green-vegetable smoothie. It was intensely delicious in a way that set off alarm bells. RFD wouldn’t provide precise information about the ingredients, but I found a recipe online for “Tofu ‘Fish’ Cakes,” which seem very close to what I ate. Essentially, they consist of some tofu mixed with a lot of refined carbs (the RFD version contains at least some unrefined carbs) along with oil and soy milk, all fried in oil and served with a soy-and-oil-based tartar sauce. (Tofu and other forms of soy are high in protein, but per 100 calories, tofu is as fatty as many cuts of beef.) L.A. being to the wholesome-food movement what Hawaii is to Spam, I ate at two other mega-popular wholesome-food restaurants while I was in the area. At Café Gratitude I enjoyed the kale chips and herb-cornmeal-crusted eggplant parmesan, and at Akasha I indulged in a spiced-lamb-sausage flatbread pizza. Both are pricey orgies of fat and carbs.
These changes often result in appetite reduction, increased satiety and a decline in the natural appreciation of food. Collectively, these conditions contribute to a condition referred to as the “anorexia of aging.” To further exacerbate the problem, older adults show a reduced ability to adapt to periods of under- or overeating. They gain or lose weight quickly, and do not easily return to their original weight following such periods. This makes the elderly population much more susceptible to unintended (and lasting) changes weight.
The National Institute on Aging, part of the National Institutes of Health, recommends four types of exercises that older Americans should include in their workout: endurance activities, such as walking, biking, or swimming; strength training, such as light weightlifting, to reduce age-related muscle loss; stretching, to maintain flexibility; and balance exercises, to reduce the likelihood of falls.
The baby-boomer weight crisis is rapidly moving past their burden of buying plus-size clothing. Modern medicine will keep them alive longer than previous generations even with their increased need for care due to their weight gain. Their increased health care cost will push America into a debt crisis. One driver will be paying for extended medical care to medicate the life choices made by the boomer generation. The second driver will be the increased gross amounts the boomer generation will be paid from Social Security by living longer.

High blood pressure and high cholesterol, which are risk factors for cardiovascular disease (CVD). In one study, 70 percent of obese children had at least one CVD risk factor, while 39 percent had two or more.
Most of the data about whether avoiding weight gain or losing weight reduces cancer risk comes from cohort and case-control studies. As with observational studies of obesity and cancer risk, these studies can be difficult to interpret because people who lose weight or avoid weight gain may differ in other ways from people who do not.
Asthma and obstructive sleep apnea are two common respiratory diseases that have been linked with obesity. In a meta-analysis of seven prospective studies that included 333,000 subjects, obesity increased the risk of developing asthma in both men and women by 50 percent. (33) Obesity is also a major contributor to obstructive sleep apnea (OSA), which is estimated to affect approximately one in five adults; one in 15 adults has moderate or severe obstructive sleep apnea. This condition is associated with daytime sleepiness, accidents, hypertension, cardiovascular disease, and premature mortality. Between 50 percent and 75 percent of individuals with OSA are obese. (32) Clinical trials suggest that modest weight loss can be helpful when treating sleep apnea. (34, 35)

Being underweight can be especially serious for older people. It increases your risk of health problems, including bone fracture if you fall. It weakens your immune system, leaving you more susceptible to infections, and it increases your risk of being deficient in important nutrients such as vitamins and minerals.
Asthma is a condition in which hyperreactive airways constrict and result in symptoms like wheezing, coughing, and shortness of breath. Causes of asthma include genetics, environmental factors, personal history of allergies, and other factors. Asthma is diagnosed by a physician based on a patient’s family history and results from lung function tests and other exams. Inhaled corticosteroids (ICS) and long-acting bronchodilators (LABAs) are used in the treatment of asthma. Generally, the prognosis for a patient with asthma is good. Exposure to allergens found on farms may protect against asthma symptoms.
May qualify for Gastric Balloon. This BMI range may also qualify for other procedures if the patient has poorly controlled type 2 diabetes, a higher risk of cardiovascular disease, or suffers from another weight-related health issue.

Orlistat can be taken up to three times a day, with each fat-containing meal. The drug may be taken during the meal or up to one hour after the meal. If the meal is missed or is very low in fat content, the medications should not be taken.
The rise in obesity among 40-to-64-year-olds accounts for much of the rise in both disability and physical-function limitations, according to Martin’s analysis. Apple-shaped people whose fat is concentrated mostly in the abdomen are more likely to develop many of the health problems associated with obesity. They are at increased health risk because of their fat distribution. While obesity of any kind is a health risk, it is better to be a pear than an apple.
“Weight gain is occurring, but it’s not just because of aging,” says Wojtek Chodzko-Zajko, head of the Department of Kinesiology and Community Health at the University of Illinois at Urbana-Champaign. “It’s what we eat, our lifestyle habits, how much screen time we get … all of those kinds of factors are having an impact.”
Your doctor may diagnose overweight and obesity based on your medical history, physical exams that confirm you have a high body mass index (BMI) and possibly a high waist circumference, and tests to rule out other medical conditions.
The most likely culprits for weight gain and obesity are food intake and sedentary lifestyles, though genetic factors can also play a role. Sweetened beverages (sodas and juices) and potato chips are two of the biggest contributors, along with our alarmingly-large portion sizes. Ever-increasing access to fast food and processed foods also play a major role.Sedentary lifestyles are also a major cause of obesity and weight gain. And while our society has become more sedentary as a whole, seniors – already less active than other age groups –feel the impact on their waistlines even more.
The branch of medicine that deals with the study and treatment of obesity is known as bariatrics. As obesity has become a major health problem in the United States, bariatrics has become a separate medical and surgical specialty.
Obesity and Stroke. Ischemic (clot-caused) stroke and coronary artery disease share many of the same disease processes and risk factors. A meta-analysis of 25 prospective cohort studies with 2.3 million participants demonstrated a direct, graded association between excess weight and stroke risk. Overweight increased the risk of ischemic stroke by 22 percent, and obesity increased it by 64 percent. There was no significant relationship between overweight or obesity and hemorrhagic (bleeding-caused) stroke, however. (10) A repeat analysis that statistically accounted for blood pressure, cholesterol, and diabetes weakened the associations, suggesting that these factors mediate the effect of obesity on stroke.
The BMI is a measure of your weight in relation to your height, and your waist circumference measures your abdominal fat. Combining these with information about your additional risk factors will give you an idea of your risk for developing obesity-associated diseases.
Non-prescription orlistat (Alli). Orlistat inhibits fat absorption in the intestine. Until recently, this medication was only available by prescription (Xenical). The over-the-counter medicine is sold at a lower dose than Xenical. But the active ingredient is the same.

Heart attack. A prospective study found that the risk of developing coronary artery disease increased three to four times in women who had a BMI greater than 29. A Finnish study showed that for every 1 kilogram (2.2 pounds) increase in body weight, the risk of death from coronary artery disease increased by 1%. In patients who have already had a heart attack, obesity is associated with an increased likelihood of a second heart attack.
It is not necessary to achieve an “ideal weight” to derive health benefits from obesity treatment. Instead, the goal of treatment should be to reach and hold to a “healthier weight.” The emphasis of treatment should be to commit to the process of lifelong healthy living, including eating more wisely and increasing physical activity.
Another dynamic may be at work, said Martin. Not only could the reported increases in disability reflect actual deteriorating health, but it could also reflect greater awareness of health conditions in the wake of more widespread diagnosis and treatment.

Begin increasing your activity level. Try to get up and move around your home more frequently. Start gradually if you aren’t in good shape or aren’t used to exercising. Even a 10-minute daily walk can help. If you have any health conditions, or if you’re a man over age 40 or a woman over age 50, wait until you’ve talked to your doctor or health care provider before you start a new exercise program.
Mirtazapine (Remeron), a serotonin antagonist used to treat depression, has gained interest as a possible treatment for unintentional weight loss in older patients because 12% of patients who take this drug for depression report weight gain.36 Although no literature exists to support its use for unintentional weight loss, mirtazapine may be an option for older patients with depression who also have unintentional weight loss. Because dizziness and orthostatic hypotension are possible adverse effects of mirtazapine, caution is warranted in patients at risk of falls.36,37

“In older, obese people, it may be more important to improve physical function and quality of life, rather than to reverse or treat risk factors for cardiovascular disease,” says Villareal, now chief of geriatrics at the New Mexico Veterans Affairs Health Care System and professor of medicine at the University of New Mexico School of Medicine, both in Albuquerque. “Combining exercise and weight loss isn’t designed so much to extend their life expectancy as it is to improve their quality of life during their remaining years and to help seniors avoid being admitted to a nursing home.”
“Of the ranked diets, both DASH and the Mediterranean diet can help people with both diabetes prevention and management,” Campbell says. They work because they encourage a variety of foods and make people aware of the carbs they consume, she adds. Both diets are mentioned in the latest nutrition guidelines ​from the American Diabetes Association.
Bittman is hardly alone in his reflexive dismissals. No sooner had McDonald’s and Burger King rolled out their egg-white sandwich and turkey burger, respectively, than a spate of articles popped up hooting that the new dishes weren’t healthier because they trimmed a mere 50 and 100 calories from their standard counterparts, the Egg McMuffin and the Whopper. Apparently these writers didn’t understand, or chose to ignore, the fact that a reduction of 50 or 100 calories in a single dish places an eater exactly on track to eliminate a few hundred calories a day from his or her diet—the critical threshold needed for long-term weight loss. Any bigger reduction would risk leaving someone too hungry to stick to a diet program. It’s just the sort of small step in the right direction we should be aiming for, because the obese are much more likely to take it than they are to make a big leap to wholesome or very-low-calorie foods.
If you’re struggling to be interested in food or you’ve lost the motivation to eat, try to eat with friends or family as often as possible. Lunch clubs are also a great way to make mealtimes more social.

In Sacramento, 56 percent of obese baby boomers have high blood pressure, the UCLA figures show, compared with 23 percent of boomers with a normal body weight. More than one-fifth of obese baby boomers in the region have diabetes. Forty percent suffer from arthritis: Not surprisingly, the number of boomers using assistive devices, such as canes and walkers, is on the rise, as well. Almost 20 percent of obese boomers can’t work due to disability.
Follow a healthy eating plan. Focus on low-calorie, nutrient-dense foods, such as fruits, vegetables and whole grains. Avoid saturated fat and limit sweets and alcohol. Eat three regular meals a day with limited snacking. You can still enjoy small amounts of high-fat, high-calorie foods as an infrequent treat. Just be sure to choose foods that promote a healthy weight and good health most of the time.

Unintentional weight loss of more than 4% in a year appears to be an independent predictor of increased mortality (relative risk [RR] 2.43, 95% CI 1.34–4.41).4 In a prospective study of 41 836 women, conducted in the United States as part of the Iowa Women’s Health Study, one or more episodes of unintentional weight loss of more than 20 pounds during adulthood was associated with a 46%–57% higher rate of death.29 A prospective study of 4869 male patients older than 65 years from general practices in 24 towns across the United Kingdom found that unintentional weight loss was associated with higher mortality risk only among those with cancer (adjusted relative risk [ARR] 1.71, 95% CI 1.33–2.19) after adjustment for lifestyle characteristics and pre-existing disease.30 A retrospective chart review of 148 long-term care residents residing in the southeastern United States found that those who lost 5% or more of their body weight within one month were 4.6 times more likely to die within one year.31

Given that many older adults suffer from obesity, you will likely have some of these clients coming to you for help in losing fat and increasing muscle mass and strength. Of course, strength training along with sensible eating can be instrumental in bringing about desired changes in overall body composition. Using the workouts in chapter 4, you can easily adjust training loads or resistances to match current strength levels while selecting exercises that can be performed safely on sturdy and properly sized machine and free-weight equipment.

Another randomized crossover study (n = 34) looked at the provision of breakfast food for dinner to patients with dementia, given that breakfast food is generally well consumed.20 Consumption of nutritional supplements between breakfast and dinner was also encouraged. There was no significant change in mean body weight with the intervention.
More than one-third of people age 65 and older in the United States are obese, according to the study authors. Obesity worsens the typical age-related decline in physical functioning and causes frailty, while weight loss can lead to harmful declines in muscle mass and bone density.

It’s important for your senior to maintain body weight once they are advanced in age, experiencing frailty, or undergoing treatments for cancer. Weight loss for seniors who are advanced in age is typically not recommended, and rapid weight loss in older adults can indicate a medical problem, malnourishment, or an issue with ease of eating or dental problems.
Fortunately, researchers are beginning to understand differences between the wrong mix and a healthy one, as well as the specific factors that shape those differences. They hope to learn how to cultivate this inner ecosystem in ways that could prevent—and possibly treat—obesity, which doctors define as having a particular ratio of height and weight, known as the body mass index, that is greater than 30. Imagine, for example, foods, baby formulas or supplements devised to promote virtuous microbes while suppressing the harmful types. “We need to think about designing foods from the inside out,” suggests Jeffrey Gordon of Washington University in St. Louis. Keeping our gut microbes happy could be the elusive secret to weight control.

But when it comes to disease fears, the baby boomers are misguided. Nearly half of them worry most about cancer, the second leading killer in the U.S.. Heart disease is the nation’s No. 1 killer, but it’s third in line on the boomers’ worry list. They’re more concerned about memory loss.
A crude population measure of obesity is the body mass index (BMI) which is a simple index of weight-for-height that is commonly used in classifying overweight and obesity in adult populations and individuals – a person’s weight in kilograms is divided by the square of the height in meters (kg/m2). BMI provides the most useful population-level measure of overweight and obesity as it is the same for both sexes and for all ages of adults but it is merely a rough guide because it may not correspond to the same degree of fatness in different individuals.
Unfortunately, it’s common to regain weight no matter what obesity treatment methods you try. If you take weight-loss medications, you’ll probably regain weight when you stop taking them. You might even regain weight after weight-loss surgery if you continue to overeat or overindulge in high-calorie foods. But that doesn’t mean your weight-loss efforts are futile.
Nearly 70 percent of adults over age 60 are overweight or obese, putting them at higher risk of diabetes and other diseases, according to a new study in the Journal of the American Medical Association.
Know and avoid the food traps that cause you to eat. Identify situations that trigger out-of-control eating. Try keeping a journal and write down what you eat, how much you eat, when you eat, how you’re feeling and how hungry you are. After a while, you should see patterns emerge. You can plan ahead and develop strategies for handling these types of situations and stay in control of your eating behaviors.

Individual responsibility can only have its full effect where people have access to a healthy lifestyle. Therefore, at the societal level it is important to support individuals in following the recommendations above, through sustained implementation of evidence based and population based policies that make regular physical activity and healthier dietary choices available, affordable and easily accessible to everyone, particularly to the poorest individuals. An example of such a policy is a tax on sugar sweetened beverages.
Nevertheless, the follow-up study of weight and breast cancer in the Women’s Health Initiative (36) found that for women who were already overweight or obese at baseline, weight change (either gain or loss) was not associated with breast cancer risk during follow-up. However, for women who were of normal weight at baseline, gaining more than 5% of body weight was associated with increased breast cancer risk.
Almost any of the commercial weight-loss programs can work but only if they motivate you sufficiently to decrease the amount of calories you eat or increase the amount of calories you burn each day (or both). What elements of a weight-loss program should a consumer look for in judging its potential for safe and successful weight loss? A responsible and safe weight-loss program should be able to document for you the five following features:
Fewer studies have examined possible associations between weight loss and cancer risk. Some of these have found decreased risks of breast, endometrial, colon, and prostate cancers among people who have lost weight. However, most of these studies were not able to evaluate whether the weight loss was intentional or unintentional (and possibly related to underlying health problems).
Americans are consuming more calories on average than in past decades. The increase in calories has also decreased the nutrients consumed that are needed for a healthy diet. This behavioral problem also relates to the increase in portion sizes at home and when dining out.
In some cases, weight-loss surgery, also called bariatric surgery, is an option. Weight-loss surgery limits the amount of food you’re able to comfortably eat or decreases the absorption of food and calories or both. While weight-loss surgery offers the best chance of losing the most weight, it can pose serious risks.
Your whole body feels it, from your joints to your heart, blood pressure, blood sugar, and other systems. The extra fat cells produce inflammation and various hormones, which boosts your odds of chronic medical conditions.
In addition to suffering from poor physical health, overweight and obese children can often be targets of early social discrimination. The psychological stress of social stigmatization can cause low self-esteem which, in turn, can hinder academic and social functioning, and persist into adulthood. While research is still being conducted, there have been some studies showing that obese children are not learning as well as those who are not obese. Further, physical fitness has been shown to be associated with higher achievement.
Psychological conditions that contribute to weight loss must be addressed promptly, as they can result in rapid loss of body mass if left untreated. Counselling, therapy and perhaps medication may be required to mitigate the effects of the mental condition.

There are many senior food assistance programs available to older adults on a limited or fixed income to help with food delivery, including Meals on Wheels, Elderly Nutrition Program, and the Home-Delivered Nutrition program, which serves frail, homebound or isolated individuals who are age 60 or over. The Congregate Nutrition Program creates an environment for older adults to eat a congregate meal. Sharing meals with other adults can help increase food intake and food enjoyment, which benefits senior nutrition overall.

Physiological influences: Some researchers believe that every person has a predetermined weight that the body resists moving away from. Also, people of the same age, sex and body size often have different metabolic rates. This means their bodies burn food differently.
