In the United States, the number of obese older adults has reached disturbing heights—now affecting approximately 20 percent of those ages 65 and older—and is only expected to rise as more “baby boomers” become senior citizens.

Pregnancy. During pregnancy, a woman’s weight necessarily increases. Some women find this weight difficult to lose after the baby is born. This weight gain may contribute to the development of obesity in women.

The area I found that’s most chockablock with commercial food options brackets the busy intersection of two main streets. Before we look at ways to beat the bulge, it is time to get real. Dogs are fatter than ever. It’s estimated that 53% of all dogs in the US are overweight or obese. That’s more than 40 million dogs. There are so many overweight dogs in the world that when we see a dog that is at a healthy weight, we immediately think she is too skinny and unhealthy.
Osteoarthritis is a common joint condition that most often affects the knee, hip, or back. Carrying extra pounds places extra pressure on these joints and wears away the cartilage (tissue cushioning the joints) that normally protects them.

Do you have a weight problem? If you do you are not alone. An average American gains between one-half pound to one pound every year. According to some estimates, almost one out of every 3 adults in the United States (about 97 million people) are classified as overweight or obese.
According to the National Institute of Health, the percentage of those seniors entering nursing homes who are moderate and severely obese — with a body mass index of 35 or greater — has risen sharply, to nearly 25% in 2010 from 14.7% in 2000, according to a recent study, and many signs suggest the upward trend is continuing.
But when his wife, Elena Acosta, signed up for a diabetes prevention program at the local YMCA, Acosta had no choice but to participate too. “Whatever I have to eat during this program, you’re eating too,” she told him.
Much of the food isn’t all that different from what I can get in any other supermarket, but sprinkled throughout are items that scream “wholesome.” One that catches my eye today, sitting prominently on an impulse-buy rack near the checkout counter, is Vegan Cheesy Salad Booster, from Living Intentions, whose package emphasizes the fact that the food is enhanced with spirulina, chlorella, and sea vegetables. The label also proudly lets me know that the contents are raw—no processing!—and that they don’t contain any genetically modified ingredients. What the stuff does contain, though, is more than three times the fat content per ounce as the beef patty in a Big Mac (more than two-thirds of the calories come from fat), and four times the sodium.

Weight loss medicines are not recommended as a single treatment for weight loss. These medicines can help you lose weight but when combined with lifestyle changes may result in greater weight loss. Your program should include plans for weight maintenance after the weight-loss phase is over. It is of little benefit to lose a large amount of weight only to regain it. Weight maintenance is the most difficult part of controlling weight and is not consistently implemented in weight-loss programs. The program you select should include help in permanently changing your dietary habits and level of physical activity, and to alter a lifestyle that may have contributed to weight gain in the past. Your program should provide behavior modification help, including education in healthy eating habits and long-term plans to deal with weight problems. One of the most important factors in maintaining weight loss appears to be increasing daily physical activity. Try to be more active throughout the day and incorporate some simple calorie burners into your everyday routine. Even the most basic activities (such as taking an after-dinner walk, using the stairs at the mall or office instead of taking an escalator or elevator, park your car farther away so you have a longer walk) can get you prepared for more regular exercise like walking or jogging. You may choose to incorporate an individually tailored exercise program into your schedule.
Obesity is a topic of significant importance at medical schools and other programs for healthcare professionals. Excess weight, especially obesity, diminishes almost every aspect of health, from reproductive and respiratory function to memory and mood. Obesity increases the risk of several debilitating, and deadly diseases, including diabetes, heart disease, and some cancers. It does this through a variety of pathways, some as straightforward as the mechanical stress of carrying extra pounds and some involving complex changes in hormones and metabolism. Obesity decreases the quality and length of life, and increases individual, national, and global healthcare costs. The good news, though, is that weight loss can curtail some obesity-related risks. (1) Losing as little as 5 to 10 percent of body weight offers meaningful health benefits to people who are obese, even if they never achieve their “ideal” weight, and even if they only begin to lose weight later in life.
Table 1 summarizes the ten trials that met our inclusion criteria (Villareal 2006a; Villareal 2006b; Villareal 2008; Frimel 2008; Lambert 2008; Shah 2009; Villareal 2011a; Armamento-Villareal 2012; Shah 2011; Kelly 2011). Figure 2 is a schematic representation of the inter-relationships of the discussed in these trials. Three papers by Villareal et al. (two in 2006 and one in 2008) reported on the same cohort of 27 participants. The participants were sedentary (≤ 2 exercise sessions per week); with stable body weight (± 2kg) during the preceding year; unchanged medications regimes for at least six months; and mild to moderate frailty as measured by the Physical Performance Test (Brown 2000). The intervention consisted of both diet and exercise (lifestyle intervention). Energy deficit was 500–700 kcal/day supplemented with a daily multivitamin and counseling to consume adequate dietary calcium and vitamin D. The goal was 10% weight loss over the six-month intervention and weight maintenance for an additional six months. Exercise sessions consisted of 90 minutes of aerobic and resistance exercises, three days per week, at a moderate intensity (~75% peak heart rate) and progressed to 80–90% of peak heart rate. Resistance exercise started at 65% of one repetition maximum (1RM) and progressed to ~80% of 1RM.
Being significantly overweight places extra strain on all of the systems of the body, affecting them in different ways. Examples of the effects of obesity on some body systems are listed in the table below.
Physical illnesses may also have secondary characteristics that lead to weight loss. For example, gall stones often result in decreased appetite and nausea, which causes them to avoid high-energy foods.
It’s been proven that obesity puts severe strain on your dog’s body and will contribute to bone and joint problems like arthritis and hip dysplasia. It’s important for your senior to maintain body weight once they are advanced in age, experiencing frailty, or undergoing treatments for cancer. Weight loss for seniors who are advanced in age is typically not recommended, and rapid weight loss in older adults can indicate a medical problem, malnourishment, or an issue with ease of eating or dental problems.
Brain. Several medicines change the way the brain regulates the urge to eat, which can help to decrease appetite. Some examples of these medicines are diethylpropion, phendimetrazine, lorcaserin, naltrexone/bupropion, and liraglutide.

Appropriate nutritional counseling through referral to a registered dietitian is recommended to ensure that the older adult’s daily nutritional requirements are met during weight-loss programs. The benefits and risks of weight reduction in older adults should be carefully considered. Loss of lean body mass, which is already diminished in older adults, may not be appropriate in persons over 65 years of age because the loss of fat-free mass in older adults is associated with significant morbidity and mortality (Flood & Newman, 2007). A weight loss program that minimizes muscle and bone loss is recommended for the older adult who is obese and who has functional impairments or metabolic complications that might be improved by weight loss (Villareal et al., 2005). This is best achieved through a moderate reduction in daily calorie intake (500-750 kcal/d). Appropriate nutritional counseling through referral to a registered dietitian is recommended to ensure that the older adult’s daily nutritional requirements are met during weight-loss programs. It is important that the diet continue to contain 1.0g/kg of protein and include 1500mg Ca/d, as well as 1000 IU vitamin D/d (Villareal et al.).

Physical inactivity, in turn, has rapid profound effects on skeletal muscle metabolism. Unlike the common association of obesity with increased lean body mass and muscle volume in young adults, obese older individuals often develop sarcopenia, reflected by reduction in lean body mass. A recent study of 2,982 subjects, aged 70–79 years, followed up for 6.5 years, revealed that high adiposity increased the risk of new-onset mobility limitation by 40–50% (33). This study was a follow up of a one-year lifestyle intervention (Villareal 2011a). The participants remained in the community, with no contact by study personnel, until the 30-month follow-up point. The investigators recruited the first half of the participants who were randomized to the weight loss group (n=13) and diet plus exercise group (n=13) from this previously reported life-style intervention (Villareal 2011a). Of the potential participants available for recruitment, ten (38%) were lost to follow-up. The remaining sixteen participants recruited into the study were representative of the original cohort with regard to age, gender, and other demographic characteristics. Outcomes of interest in the follow-up study were changes in body weight and composition, physical function, quality of life, insulin sensitivity, BMD, and renal and liver function. Participants also completed the Block Brief 2000 Food Frequency Questionnaire (FFQ) to quantify their average daily energy intake over the previous year. Participants were included if they completed at least three days of food records, submitted the FFQ, and had daily energy intakes of more than 500 kcal per day for women, and 800 kcal per day for men. At the 30-month follow-up compared to baseline, weight (101.5 ± 3.8 vs 94.5 ± 3.9 kg) and BMI (36.0 ± 1.7 vs 33.5 ± 1.7 kg/m2) remained significantly below baseline (all p<0.05). Fat free mass (56.7 ± 2.1 vs 56.9 ± 2.2 kg) and appendicular lean mass (24.1 ± 1.0 vs 24.1 ± 1.1kg) remained unchanged when compared to the 12-month point (end of trial) and the 30-month follow-up (all p>0.05). Improvements in the physical performance test (PPT 27 ± 0.7 vs 30.2 ± 0.6), insulin sensitivity (4.1 ± 0.8 vs 3.0 ± 0.6), and insulin area under the curve (12484 ± 2042 vs 9270 ± 1139 min.mg/dl) remained unchanged at 30 months compared to baseline (all p<0.05). Waist circumference and systolic blood pressure remained lower at 30 months compared to baseline (all p<0.05). Whole body and lumbar spine BMD did not change; however, total hip BMD progressively decreased from baseline to 30 months (0.985 ± .026 vs 0.941 ± .024 g/cm2; p<0.05). There were no adverse effects on liver or renal function. Thirteen participants met inclusion requirements for the dietary analysis. At baseline the average caloric intake was 2045 ± 178 kcal per day. At the 30-month follow-up, the FFQ estimated mean daily intake was 1427 ± 142 kcal per day. Overall, participants consumed an average of 619 ± 157 kcal per day less at 30 month follow-up compared to baseline (p<0.05). But it initially wasn’t charged that way. Results from the National Health and Nutrition Examination Survey (NHANES) showed that in 2011–2014, nearly 70% of U.S. adults age 20 years or older were overweight or obese and more than one-third (36.5%) were obese (3). Surgical procedures of the upper gastrointestinal tract are collectively called bariatric surgery. The initial surgeries performed were the jejunocolic bypass and the jejunoileal bypass (where the small bowel is diverted to the large bowel, bypassing a lot of the surface area where food would have been absorbed). These procedures were fraught with problems and are no longer performed. Currently, procedures used include making the stomach area smaller or bypassing the stomach completely.

Gastric bypass surgery is a procedure that creates a small stomach pouch to restrict food intake and constructs bypasses of the duodenum and parts of the small intestine to decrease one’s ability to absorb nutrients from food. There are two types of gastric bypass operations: Roux-en-Y and extensive gastric bypass. Patients who undergo gastric bypass surgery will need to take nutrition supplements due to limited absorption of certain vitamins and minerals.

“The food they’re cooking is making people sick,” Pollan has said of big food companies. “It is one of the reasons that we have the obesity and diabetes epidemics that we do … If you’re going to let industries decide how much salt, sugar and fat is in your food, they’re going to put [in] as much as they possibly can … They will push those buttons until we scream or die.” The solution, in his view, is to replace Big Food’s engineered, edible evil—through public education and regulation—with fresh, unprocessed, local, seasonal, real food.

The one fast-food restaurant near that busy L.A. intersection otherwise filled with bodegas was a Carl’s Jr. I went in and saw that the biggest and most prominent posters in the store were pushing a new grilled-cod sandwich. It actually looked pretty good, but it wasn’t quite lunchtime, and I just wanted a cup of coffee. I went to the counter to order it, but before I could say anything, the cashier greeted me and asked, “Would you like to try our new Charbroiled Atlantic Cod Fish Sandwich today?” Oh, well, sure, why not? (I asked her to hold the tartar sauce, which is mostly fat, but found out later that the sandwich is normally served with about half as much tartar sauce as the notoriously fatty Filet-O-Fish sandwich at McDonald’s, where the fish is battered and fried.) The BMI measurement, however, poses some of the same problems as the weight-for-height tables. Not everyone agrees on the cutoff points for “healthy” versus “unhealthy” BMI ranges. A new appreciation for the impact of gut microbes on body weight has intensified concerns about the profligate use of antibiotics in children. Blaser has shown that when young mice are given low doses of antibiotics, similar to what farmers give livestock, they develop about 15 percent more body fat than mice that are not given such drugs. Antibiotics may annihilate some of the bacteria that help us maintain a healthy body weight. “Antibiotics are like a fire in the forest,” Dominguez-Bello says. “The baby is forming a forest. If you have a fire in a forest that is new, you get extinction.” When Laurie Cox, a graduate student in Blaser’s laboratory, combined a high-fat diet with the antibiotics, the mice became obese. “There’s a synergy,” Blaser explains. He notes that antibiotic use varies greatly from state to state in the U.S., as does the prevalence of obesity, and intriguingly, the two maps line up—with both rates highest in parts of the South.

If current trends continue, Australia’s Generation X will overtake Baby Boomers for poor health, including rates of obesity and diabetes, which could have huge implications for healthcare and the workforce. Baby boomers are currently in mid-life and over the next several decades they will swell the ranks of those aged 65 and over. Their entry into this age group will have a significant impact in a number of areas but particularly in relation to the type and extent of health services required. Obesity is a major health issue for this cohort as its members are significantly over-represented in both the overweight and obese categories compared to the rest of the population. In addition, they are significantly more likely to have multiple risk factors. This review considers how alterations to lifestyle, initiated by the rapid social changes of the last half century, might have contributed to obesity within this cohort. In providing this broad overview it focuses on how increased affluence and changes to everyday institutions have affected the cultures around food consumption. This includes a consideration of both the internal and external ways in which eating environments are now constructed. This review suggests that further research is needed to identify the factors which facilitate or constrain healthy ageing in the baby boom cohort. Research along these lines also needs to consider both macro- and micro-level changes to the social context within which these factors arise. This is essential as the high levels of obesity in this cohort may reflect both an individual and a structural lag in adapting lifestyles and policies to meet the needs of this very different social environment.

The interaction between diet and gut bacteria can predispose us to obesity from the day we are born, as can the mode by which we enter the world. Studies have shown that both formula-fed babies and infants delivered by cesarean section have a higher risk for obesity and diabetes than those who are breast-fed or delivered vaginally. Working together, Rob Knight of the University of Colorado Boulder and Maria Gloria Dominguez-Bello of N.Y.U. have found that as newborns traverse the birth canal, they swallow bacteria will later help them digest milk. C-section babies skip this bacterial baptism. Babies raised on formula face a different disadvantage: they do not get substances in breast milk that nurture beneficial bacteria and limit colonization by harmful ones. According to a recent Canadian study, babies drinking formula have bacteria in their gut that are not seen in breast-fed babies until solid foods are introduced. Their presence before the gut and immune system are mature, says Dominguez-Bello, may be one reason these babies are more susceptible to allergies, asthma, eczema and celiac disease, as well as obesity.

An early hint that gut microbes might play a role in obesity came from studies comparing intestinal bacteria in obese and lean individuals. In studies of twins who were both lean or both obese, researchers found that the gut community in lean people was like a rain forest brimming with many species but that the community in obese people was less diverse—more like a nutrient-overloaded pond where relatively few species dominate. Lean individuals, for example, tended to have a wider variety of Bacteroidetes, a large tribe of microbes that specialize in breaking down bulky plant starches and fibers into shorter molecules that the body can use as a source of energy.

Some people have less access to stores and supermarkets that sell healthy, affordable food such as fruits and vegetables, especially in rural, low-income neighborhoods and communities of color. Through its growing sway over health-conscious consumers and policy makers, the wholesome-food movement is impeding the progress of the one segment of the food world that is actually positioned to take effective, near-term steps to reverse the obesity trend: the processed-food industry. Even if you have one or more of these risk factors, it doesn’t mean that you’re destined to become obese. You can counteract most risk factors through diet, physical activity and exercise, and behavior changes.
Restricting certain foods. Certain diets limit the amount of a particular food group, such as high-carbohydrate or full-fat foods. Ask your doctor which diet plans have been found effective and which might be helpful for you. Drinking sugar-sweetened beverages is a sure way to consume more calories than you intended, and limiting these drinks or eliminating them altogether is a good place to start cutting calories.

Vagal nerve blockade is another treatment for obesity. The history should also identify prescription and over-the-counter medications and herbal supplements that may be affecting appetite or contributing to weight loss. A social history focusing on alcohol and tobacco use and the patient’s living situation may elicit further useful information. Body fat percentage is difficult to measure accurately, however. Special equipment is needed that is not found at most medical offices.
